Notice is hereby given that sealed Proposals will be received by the Madison County Health Department, (MCHD) for the new construction of a pole structure vehicle garage. The project will be constructed using one General Contract between the Owner and General Contractor. Bidders shall enclose with each bid Indiana State Board of Accounts Form No. 96 (current version) and a completed Non-Collusion Affidavit. A bid security in the form of AIA Document A310 or a certified check, shall accompany the bid. The bid security shall be in the amount of ten percent of the total bid (10%), including all additive alternates.. The Bid Bond shall be for the largest contract amount that could be awarded the Bidder. Bid Security shall be forfeited if bid is withdrawn after closing time on the date for receiving bids. The successful bidder is required to furnish a Performance and Labor & Material Payment Bond from an acceptable surety in an amount equal to one hundred twenty five percent (125%)) of the full contract sum. Bidding Documents may be examined in the offices of the Architect. The Owner reserves the right to reject all bids, and to waive informalities, irregularities, and errors in the bidding to the extent permitted by law. This includes the right to extend the date and time for the receipt of bids. No bid may be withdrawn after the scheduled closing time for receipt of bids for a period of sixty (60) days. All out-of-state corporations must have a Certificate of Authority to do business in Indiana. Application forms may be obtained by contacting the Secretary of State, Corporation Division.
