The bid proposal must cover all labor, equipment, materials, construction survey, traffic control, stakeout and all other costs incidental to the work. In general, the work to be performed shall consist of the replacement and installation of (1) sanitary sewer manhole, live line sewer repairs including replacement of approximately 100 L.F. of 8" PVC gravity sewer pipe, ten (10) live sewer lateral repairs and replacement, appurtenances and final restoration. Bids will be received for a single prime contract. Bids shall be on a lump sum and unit price basis as indicated in the Bid Form. Checks shall be made payable to RJD Engineering, Inc. Addenda, if any, will be issued to only those persons who are on record as having obtained a set of Contract Documents. Bidding Documents, Drawings and Technical Specifications will also be on file and open to the public for examination at the aforesaid Office of the Municipal Authority of Hazle Township, 79 Harleigh Boulevard, Harleigh, PA 18225, Monday through Friday between the hours of 8:00 AM and 4:30 PM. Upon Issuing Offices receipt of payment, printed copies of the Bidding Documents will be sent via the prospective Bidder's delivery method of choice; the shipping charge will depend on the shipping method chosen and shall be paid for by the Bidder. The date that the Bidding Documents are transmitted by the Issuing Office will be considered the prospective Bidders date of receipt of the Bidding Documents. Partial sets of Bidding Documents will not be available from the Issuing Office. Neither the Owner nor Engineer will be responsible for full or partial sets of Bidding Documents, including addenda if any, obtained from sources other than the Issuing Office. All Bids shall be addressed to Mr. Eugene Rafalli, Chairman, Municipal Authority of Hazle Township and shall be clearly labeled as "Bid Proposal for Hollywood Sewer Rehabilitation and Replacement Project Phase II". A certified check or Bid Bond drawn to the order of the Municipal Authority of Hazle Township in an amount equal to ten percent (l0%) of the total bid must be submitted with each contract proposal as bid security. Bidders shall conform to all requirements of the bidding documents for this contract. Prior to award of the Contract, the successful bidder shall furnish to the Owner Certificates of Insurance, Performance Bond, Payment Bond, and other Bonds as required in the Contract Documents, executed by a satisfactory surety and Stipulations Against Liens.
