The Town of Acworth is inviting proposal submissions for road repairs resulting from rainstorm damage incurred July 29, 2021. The event has State and Federal disaster declarations and has been designated DR-4624. Generally, work will be done under the supervision of the Town and its engineer. All work will meet Federal Emergency Management Agency (FEMA) and NH Department of Environmental Services (NHDES) requirements. The scope of work for this RFP covers the initial final repairs on Derry Hill Road in Acworth including but not limited to the following: 1. Replacement of damaged culverts and head walls. 2. Placement of stone, as needed. 3. Dust control, as needed. 4. Additional work as directed by Engineer. Proposals shall include: 1. Proposers company, individual and point of contact names and contact information; 2. A description of qualifications, similar projects and FEMA experience with references; 3. A description of in-house and subcontractor rates, materials costs and estimated project time; 4. Certificate of Insurance and W-9. Criteria for proposal selection will include: 1. Demonstration of project understanding including history of site and conditions; 2. Demonstration of willingness to work with Town engineer; 3. Completeness of time and materials description; 4. References from previous similar projects; 5. Estimated cost with breakdown as appropriate.
