APN-250-0101-011 Furnishing all labor, material, tax, transportation, equipment, and services necessary for the DEL PASO HEIGHTS ES KINDERGARTEN PLAY AREA PROJECT, Board Award of Contract September 20, 2022 Notice of Award September 23, 2022 Notice to Proceed October 14, 2022 Start Date: Construction October 17, 2022 Substantial Completion Date March 10, 2023 Contract Completion Date (Closeout) May 12, 2023 Each Bid must be accompanied by one of the following forms of bidder's security: (1) cash; (2) a cashier's check made payable to the District; (3) a certified check made payable to the District; or (4) a bidder's bond executed by a California admitted surety as defined in Code of Civil Procedure section 995.120, made payable to the District in the form set forth in the Bid Documents. Such bidder's security must be in an amount not less than ten percent (10%) of the maximum amount of bid, as a guarantee that the bidder will enter into the proposed Contract, if the same is awarded to such bidder, and will provide the required performance and payment Bonds, insurance certificates and any other required documents. In the event of failure to enter into said Contract or provide the necessary documents, said security will be forfeited. Performance Bond naming District as Obligee, in a sum not less than 100 percent of the amount of the Contract, to guarantee the faithful performance of the Contract. Labor & Material Payment Bond in a sum not less than 100 percent of the amount of the Contract, to guarantee the payment to all claimants, as defined in section 9100 of the Civil Code covering wages for services engaged and of bills contracted for materials, supplies, and equipment used in the performance of the Contract. The successful bidder shall commence work with the issuance of the Notice to Proceed and per the project schedule referenced below. The District has established a liquidated damage rate of $1,000.00 per calendar day for each and every calendar day that the work remains fully uncompleted or unaccepted by the District. Pursuant to Labor Code sections 1725.5 and 1771.1, all contractors and subcontractors that wish to bid on, be listed in a bid or enter into a contract to perform public work must be registered with the Department of Industrial Relations. No bid will be accepted nor any contract entered into without proof of the contractor's and subcontractors' current registration with the Department of Industrial Relations to perform work. If awarded a Contract, the Bidder and its subcontractors, of any tier, shall maintain active registration with the Department of Industrial Relations for the duration of the Project REMOVAL AND REPLACEMENT OF APPARATUS PLAY EQUIPMENT AND ASSOCIATED PAVING. PROVIDE ADA COMPLIANT PATH OF TRAVEL TO PROJECT SITE. The Contractor shall provide erosion and sediment control measures to the following activities, but not limited to: 1. Cut and fill operations. 2. Temporary stockpiles. 3. Vehicle and equipment storage, maintenance and fueling operations. 4. Concrete, plaster, mortar and paint disposal. 5. Dust control. 6. Tracking of dirt, mud on off-site streets. 7. Pipe flushing. 8. Appropriate Erosion Controls
