Site work and paving for a sidewalk / parking lot in Ellington, Missouri. Conceptual plans call for site work for a 4,020-square-foot sidewalk / parking lot; and for paving for a 4,020-square-foot sidewalk / parking lot.
Highlands Health Care requests qualifications for architectural design & inspection services to assist in the renovation of three (3) separate clinic settings as outlined below. This project is supported by the Health Resources and Services Administration (HRSA) of the U.S. Department of Health and Human Services (HHS) as part of an award totaling $730,000 with 0% percent financed with non-governmental sources. Project 1: Renovate a portion of the Iron County Medical Clinic into the co-located Iron County Dental Clinic in Pilot Knob, Iron County, Missouri. The need consists of cutting an opening into the existing dental hallway as well as cutting through cement flooring to run the vacuum and air lines, demo the existing room, and convert it to a dental exam room. The current square footage is 2951 square feet before renovations and 3,491 after hallway conversion. Project 2: Renovate an existing building into a new Dental Clinic in Doniphan, Ripley County, Missouri . This renovation consists of a 3,319 square feet vacant building next to Doniphan Urgent Care. Plan to convert the entire building into dental exam rooms and offices. No HVAC modifications should need to be done, or modification to building exteriors. Project 3: Expansion of the parking lot at Ellington Family Clinic in Ellington, Reynolds County, Missouri. The projected square footage of additional parking lot space is 4020 square feet. Information provided to Missouri Highlands Health Care must include: 1. The specialized experience and technical competence of the firm with respect to construction design, specification preparation or related work; 2. The capacity and capability of the firm to complete the work in question, including specialized services, within a period of eighteen months (18) from the project start date; 3. The past record of performance of the firm with respects to such factors as control of costs, quality of work, and ability to meet schedules; 4. The firm's proximity to and familiarity with the areas in which the projects are located; 5. The rate for the preliminary architectural and design work; 6. References from previous clients of related work with the firm within the past five (5) years; 7. Documentation of compliance with e-Verify requirements as well as the SAM.gov disbarment rules. The firm will be selected based on the above qualifications.
