Renovation of a municipal facility in Bowling Green, Ohio. Completed plans call for the renovation of a 2,996-square-foot, one-story above grade municipal facility.

All construction services required for the renovation of 3,000 square feet of clinic spaces, including the pharmacy, waiting area/check-in, new behavioral health rooms, support spaces Renovation of waiting area, pharmacy, check-in/check-out, and associated support spaces. New Behavioral Health Treatment rooms shall be included in the reconfigured space. All areas shall receive new finishes and casework in select rooms. Work includes new lighting fixtures, ceilings, and modifications to HVAC system to conform with design. New sinks will be added in a few spaces and the sprinkler system will be adjusted to fit new layout. The work will consist of one single prime contract. The estimates of probable cost for the project is $660,000 with Alternate 1-$6,600, Alternate 2-$8,858 and Alternate 3-$98,000. Bidders shall not be responsible for paying sales taxes on materials incorporated into the structure. The Owner is tax exempt and a certificate will be provided to the successful contractor. This project is supported by the Health Resources and Services Administration (HRSA) of the U.S. Department of Health and Human Services (HHS) under grant number C8ECS44203 Health Center Infrastructure Support Program in the amount of $517,105.00. All Requests for Information (RFI) and questions must be e-mailed (no telephone calls) to hjudge@ssoe.com and received by August 26, 2022 by 10:00 a.m. local time. An Addendum will be issued August 29, 2022 by 2:00 p.m. local time in response to all received RFI's. All bids must be signed and submitted on requested bid documents. Bids must state the prices in the blanks provided and be enclosed in a sealed envelope marked Wood County Combined General Health District Interior Renovation Project and addressed to the Wood County Commissioners, One Courthouse Square, Bowling Green, Ohio 43402. The bid guaranty may be of two forms: 1. A Bid Guaranty and Contract Bond using the form in the Contract Documents. 2. A certified check, cashier's check or letter of credit in favor of Wood County Commissioners, in the amount of ten percent (10%) of the bid. If the contract is awarded, a Contract Bond will be required, which is a one hundred percent (100%) payment and performance bond. Bidders shall submit with their bid the affidavit required under the Ohio Revised Code, Section 5719.042 that the bidder was not charged with any delinquent personal property taxes in Wood County, Ohio. The Contractor declares that its principal officers, directors, shareholders and/or partners are current with any court-ordered child support payments pursuant to the Board of County Commissioners' Resolution No. 92-2041. The successful bidder is required to comply with Appendix II to 45 CFR Part 75 - Contract Provisions for Non-Federal Entity Contracts under Federal Awards. All waste materials generated from this project must be taken to the Wood County Landfill for disposal. The Board of County Commissioners shall be responsible for all hauling and landfill costs and shall provide a roll-off box for this use. Contractor shall advise the Board of County Commissioners when the roll-off box is full. The successful bidder will be required to pay not less than the minimum wage rates established by the Department of Commerce, Division of Industrial Compliance, Bureau of Wage and Hour Administration of the State of Ohio in accordance with all provisions of the Prevailing Wage Act of the State of Ohio, ORC Sections 4115.03 and 4115.16 and related requirements. Bids received after the scheduled bid opening date and time, or not accompanied by a satisfactory bid bond or check, will neither be read nor considered. Domestic steel use requirements as specified in section 153.011 of the revised code apply to this project. Copies of section 153.011 of the revised code can be obtained from the office of the ohio facilities construction commission. The Board of County Commissioners may reject any or all of the bids. The Board of County Commissioners reserves the right to waive any irregularities in the form of the bid that do not affect or destroy competitive bidding. In no case will an award be made until all necessary investigations are made as to the qualifications of the bidder to whom it is proposed to award the contract.
