Site work for a residential development in Minneapolis, Minnesota. Completed plans call for site work for a residential development.

SCOPE OF WORK/TECHNICAL SPECIFICATIONS. The MPHA is seeking bids from qualified, licensed, and insured entities to provide the following detailed services: 1.1 Required Services. MPHA requires the removal of an attached garage and associated foundation repairs at 1100 Irving Ave N. This work includes demolition of concrete driveway, garage addition, sidewalk, and stairs. We are tying into the existing foundation with a new footing and wall. There will be a new service door entrance. The Contractor will provide all material and labor to complete the construction work detailed herein and within any applicable attachments most specifically within Attachment K and K-1. All work is to be completed in strict accordance with manufacturer instructions and all federal, state, and local codes and regulations. Failure to abide by the aforementioned requirements may result in the required removal and reinstallation of materials at the Contractor's expense. 1.1.1 Scope of work to include, but is not limited to, the following: All change orders must be pre-approved before occurring or MPHA will not pay them. These can be approved via, text or email. A written change order will need to be signed by an approved MPHA's project manager and the contractor before it can be invoiced. If a change order is needed, please call the PM immediately. The PM or other representative will try to be onsite within a few hours to review the change order request. Review all scope items and performance specifications. Review all attached HUD documents. Supply photographs of all work completed before submitting your payment request. Make sure the photographs show the quality of work and label them so the PM can tell what is being presented. Contractor must let the PM know at least 48 hours before they will be starting work. Contractor will confirm a schedule with the PM prior to any work starting. Contractor will confirm their start date with the PM 2 days prior to work starting. Contractor must supply their own electricity/generator for the project. Contractor cannot use electricity from the house they're working on. If you plan to use parking pads for the dumpsters you must receive permission from the PM first. The Contractor will have 3 business days to sign their contract and return it to MPHA. Once the contract is signed and a notice to proceed is processed Contractor must start the work within 1 week. Contractor shall have three weeks to complete the work. Upon completion, Contractor will need to call the PM to review punch list items to be corrected. If the PM has to make more than 2 trips out to review the punch list items, the MPHA may deem the Contractor non-responsible when bidding on future solicitations. All permits must be pulled prior to starting any work. All items below need to be completed prior to sending MPHA a request for payment: o Permits must be closed o Certified payrolls must be entered into LCP Tracker o IC134 must be approved by the Department of Revenue and uploaded into LCP Tracker o The Contractor Sales Tax Documentation must be filled out and emailed to the PM A list of all material must be included in the final invoice including warranty information. MPHA values resident participation and employment in its contracting. MPHA has a participation goal of 20% minority-owned business enterprises (MBE), and 7% woman-owned enterprises (WBE). In addition, 25% of the total labor hours worked on MPHA projects must be worked by Section 3 workers, of which 5% must be worked by Targeted Section 3 workers (see Attachments D & E for more information). The selected Proposer agrees to comply, in writing, with all applicable equal opportunity and affirmative action laws, directives, and regulations of the federal, state, and local governing bodies or agencies thereof.
