I. PURPOSE The purpose of this solicitation is to establish a contract with one qualified source for preventive maintenance services for heating, ventilation, air conditioning (HVAC) equipment at a firm fixed price and an "on-call" repair service on a time and materials basis for the DMV Tappahannock Customer Service Center. II. SCOPE OF WORK A. Requirements: 1. The Contractor shall furnish Preventive Maintenance Service for HVAC equipment (including all necessary parts, labor, supervision, materials, and equipment) and "on-call" repair services on all heating, ventilation, and air conditioning (HVAC) equipment, water heaters, water coolers, drinking fountains, and exhaust fans. Preventive maintenance shall include all adjustments and servicing required to maintain systems and equipment in good operating condition, in accordance with the manufacturer's specifications and Attachment B, titled "General Equipment Preventive Maintenance Guidelines". 2. Bidders shall have previous experience providing maintenance, installation and repairs of hvac equipment. 3. Bidders shall complete and return pages 1, 5, 13, 16 and 17 by the response due date. 4. To schedule a site visit, please contact the CSC Manager or Assistant Manager at 804-443-3379 to arrange a site visit to review the equipment. B. Preventive MaintenanceService: 1. The Contractor shall perform preventive maintenance service on all HVAC equipment within ten (10) days of the award of the contract. The Contractor shall use the service checklist described in Attachment B as the starting point for preventive maintenance. Seasonal changeover service shall be performed when appropriate and/or as requested by DMV's CSC Manager. 2. Upon completion of the initial service of the HVAC equipment, the Contractor shall submit a written report to DMV's CSC Manager. A copy of this report shall be attached to the invoice. This report shall identify the building name or number, the date(s) service was performed and the following additional information on each piece of HVAC equipment: a. Equipment description, manufacturer, model and serial number. b. A detailed, narrative statement as to the condition of the equipment indicating work that apparently has been performed previously and an estimate of the age. c. Specific recommendations for any major repair or modification which will enhance the operation of this equipment. d. An estimate of the cost to accomplish the work recommended in item c above. e. A list of service items to include filter type, size, and number; drive belt size, type and number; any other information related to servicing the equipment. C. General 1. The Contractor shall provide "on call" repair services when requested in writing or verbally by an authorized DMV staff member. 2. The Contractor shall complete a service check log upon the completion of each service visit (Attachment C). This log provided by DMV tracks the type of preventive maintenance or repair task performed on each piece of equipment at the facility. 3. The Contractor shall deliver to the DMV's Contracting Officer all copies of warranty certificates, maintenance manuals, and other pertinent data as it relates to replaced parts and equipment. 4. The Contractor shall not include charges for items which are not listed on the pricing schedule. 5. The Contractor shall provide parts and materials equal to those of the original equipment manufacturer for repair of all HVAC equipment. PARTS AND SUPPLIES SHALL BE PROVIDED AT THE CONTRACTOR'S ACTUAL INVOICED COSTS. NO MARKUP OF PARTS OR SUPPLIES SHALL BE ALLOWED. 6. Travel charges including truck charges will not be allowed. 7. A site visit is optional prior to submitting your bid. However, bidders should inspect the site where work is to be performed to view the equipment and area(s) work is to be performed in. All job site inspections must be scheduled at a time convenient for the DMV.
