For Replace Asphalt Parking Area and Driveway CR Number: 2089809 The LI DDSOO intends to award the contract to the most responsible and responsive vendor that provides the lowest cost and meets the requirements detailed in the scope of work Upon request, vendor must demonstrate relevant experience on prior projects of similar magnitude. The LI DDSOO reserves the right to reject any and all quotes. In deciding whether to or not to submit a quote, please be aware the awarded vendor must adhere to the time frames outlined in the scope of work. The awarded vendor will also be required to submit certificates of Liability Insurance and Workers Compensation Insurance as well as completed Lobbying Law Forms, and Signed Appendix A. NYS Labor law requires that all contracted employees performing maintenance, repairs or capital improvements in state owned buildings receive the prevailing wage and supplements rate for the occupation in which they are working. If this is a public work contract covered by Article 8 of the Labor Law or a building service contract covered by Article 9 thereof, neither Contractors employees nor the employees of its subcontractors may be required or permitted to work more than the number of hours or days stated in said statutes, except as otherwise provided in the Labor Law and as set forth in prevailing wage and supplement schedules issued by the State Labor Department. Furthermore, contractor must pay at least the prevailing wage rate and pay or provide the prevailing supplements, including the premium rates for overtime pay, as determined by the State Labor Department in accordance with the Labor Law. NYS law requires the vendor to keep detailed records for each person employed including name, address, telephone number, social security number, occupational classification in which worked, hourly wage rate paid, supplements provided, number of hours worked, and actual wages paid. Certified payroll record/s MUST be submitted at the time of invoicing. Effective April 28, 2008, if this is a public work contract covered by Article 8 of the Labor Law, the contractor understands and agrees that the filing of payrolls in a manner consistent with Subdivision 3-a of Section 220 of the Labor Law shall be a condition precedent to payment by the State of any State approved sums due and owing for work done upon the project. Without submittal of certified payroll record/s, payment will not be made. OPWDD does not allow subcontracting on this type of work. Service-Disabled Veteran-Owned Set Aside: No Minority Owned Sub-Contracting Goal: 30% Women Owned Sub-Contracting Goal: 7% Service-Disabled Veteran-Owned Business Contracting Goal: 6% Contractor shall be responsible for marking out all underground power and utility lines before digging. Contractor will maintain a means of egress for the house. Remove vegetation in the way of project work. Excavate, and discard offsite the existing parking area (approx. 7,400 sqft) and debris to the sub-base material. Saw cut to meet new edge of asphalt were required. Remove existing sub-base material as required to properly grade and pitch for drainage to prevent pooling of water. Properly compact the existing sub-base. Provide 4" dense grade recycled concrete aggregate (RCA) and properly compact. Provide 2" asphalt top course (NYS DOT 6F) and properly compact. Apply tack coat to properly seal off the new driveway between existing and new installation, walkways, patios, steps or ramps. Provide soil around the edge of new asphalt to alleviate any type of drop off or tripping hazard around the edges. Rake even and seed all areas of lawn disturbed by construction. The new installed driveway and parking area is to be properly graded and pitched as to prevent any pooling of water. . Removal and disposal of debris from the facility
