The work in this contract includes the construction of approximately 120 square yards of concrete sidewalk, aprons and curb ramps, 120 square yards of concrete driveway apron, 470 linear feet of curbing, storm drainage improvements, 8,700 square yards of roadway milling, 1,100 square yards of hot mix asphalt pavement repair, 1,150 tons of hot mix asphalt pavement resurfacing, 347 linear feet of pavement markings and other incidental work. Each bid must be made on the prescribed forms furnished with the Contract Drawings and Specifications, including Proposal Form, Schedule of Prices, Bid Bond Guarantee, Site Inspection Affidavit, Acknowledgment of Receipt of Changes to Bid Documents, Stockholder Statement of Ownership, Non-Collusion Affidavit, Consent to Investigation, Bidder's Qualification Form, NJ Business Registration Certificate, Public Works Contractor Registration Certificate, List of Subcontractors, Plan and Equipment Questionnaire, Disclosure of Investment Activities in Iran, and Bid Opening Checklist. Bid must be accompanied by a Consent of Surety and a Certified Check, Cashier's Check or Bid Bond of not less than ten (10%) percent of the amount bid, and not to exceed $20,000.00.
