Remodeling of a multi-residential development in Dale, Indiana. Completed plans call for the remodel of a multi-residential development.

Dale Community Apartments, 490 West Med calf Street,Dale, IN 47523 that sealed bids for the Remodeling of the Apartments Complex will be received. Bids shall be submitted to the Dale Community Apartments office 490 West Med calf Street, Dale, Indiana 47523. The work consists of site improvements and exterior and interior remodeling of buildings consisting of General Construction, HVAC, Plumbing, and Electrical. Project location is at 490 West Med calf Street, Dale, Indiana 47523 A combined bid as listed below and called for in the specifications is to be submitted on the State Board of Accounts Form #96, Bid for Public Works. This form must be filled out in its entirety and no tarized. Also, to be submitted is the form Work Schedule, Addendum Acknowledgement, and Alternate Bid Sum. Base Bid #1 - A combined bid for all trades of work as shown on the plans and called for in the specifications shall be submitted on the State Board of Accounts Form #96, Bid for Public Works, with Work Schedule,Addendum Acknowledgement, and Alternate Bid Sum Form provided in the specifications. Each bidder must file with their sealed bid, a bid bond or certified check in the amount equal to five percent (5%) of the total bid, said bond provided by an insurance company authorized to do business in the State of Indiana, payable to Dale Community Apartments, Dale, Indiana. The above bond or check is to ensure the execution of the contract on which such bid is made. The contractor to whom the work is awarded will be required to furnish, before commencing work, performance and payment bonds in an amount equal to the bid price of the contract awarded to said contractor. Cost of said bond shall be included in the bid and will be required to remain in place for 1 year following construction completion. Contractors shall also provide certificates of all insurance required in the specifications. This project is funded by the Indiana Housing and Community Development Authority (IHCDA). Neither the United States nor the State of Indiana nor any of its departments, agencies, or employees is or will be a party to this Advertisement or Bids or any resulting contract. Recipient of Funds: Dale Community Apartments, Dale, Indiana. Consistent with Presidential Executive Orders 11625, 12138, and 12432, and Section 3 of the HUD Act of 1968, all feasible efforts shall be made to ensure that small and minority-owned businesses (MBE), womens business enterprises (WBE), and other individuals or firms located in or owned in substantial part by persons residing in the area of the public housing authority project are used when possible. In addition,the Indiana Housing & Community Development Authority has a goal of ten percent(10%) participation by MBE/WBE firms in HOME funded projects. Therefore, efforts must be made (and documented) to attract proposals from minority-owned businesses and womens business enterprises. This project is also under the jurisdiction of U.S. Department of Agriculture - Rural Development. Contractors and sub-contractors are required to pay not less than the prevailing wage rates established by the U.S. Department of Labor, Davis-Bacon Act included in the project specifications. Bidders on this work shall be required to comply with provisions of the Presidents Executive Order No. 11246, as amended. The Bidders shall also comply with the requirements of 41 CFR Part 60-4 entitled Construction Contractors-Affirmative Action Requirements. A copy of 41 CFR Part 60-4 may be found in the Supplemental General Conditions of the Contract Documents and Specifications. All contracts awarded using the Indiana Housing and Community Development Authority (IHCDA) funding from the federal Housing & Urban Development (HUD)agency, require all contractors to comply with the following appropriate Federal regulations including: Federal Contract Provisions: a) Equal Employment Opportunity - Executive Order 11246: Equal Opportunity Clause, goals for female and minority participation and implementing regulations b) Rights to Inventions Made Under a Contract or Agreement c) Clean Air Act (42 U.S.C. 7401 et seq.) and the Federal Water Pollution Control Act (33 U.S.C. 1251 et seq.), as amended d) Byrd Anti-Lobbying Amendment (31 U.S.C. 1352) e) Debarment and Suspension (Executive Orders 12549 and 12689) Construction Contracts and Subcontracts for Davis-Bacon Projects must also include the following (See Labor Standards chapter): a) Federal Wage Determination with modifications b) Federal Labor Standards Provisions (HUD 4010) i. Davis-Bacon Act ii. Contract Work Hours and Safety Standards Act iii. Copeland Anti-Kickback Act c) Contractors and Subcontractors Certifications (HUD 1421 and 1422) d) OFCCP Subcontractor Notification e) EEO-1 Report f) CC-257 Monthly Utilization Report g) U.S. Department of Labor Notices (job site): WH 1321, WH 1462, and OSHA2203 publications Section 3 requirements (See Section 3 chapter). No bidder may withdraw their bid for a period of sixty (60) days following the date set for the receiving of bids. The Owner reserves the right to retain any and all bids for this period, and said bids shall remain in full force and effect during said time. The Owner further reserves the right to waiver in formalities, and the right to reject any and all bids. The following fully executed documents shall be submitted with the bid: a. State Board of Accounts Form #96, Bid for Public Works including non collusion affidavit with Work Schedule, Addendum Acknowledgement and Alternate Bid Sum Form. b. 5% Bid Bond or Certified Check. c. Certified financial statement containing the information required in Form 96. d. List of similar past projects. e. List of sub-contractors that contractor proposes to use on this project. Award will be made to the low, responsive, responsible bidder based on the total of the base bid amount and any alternate bids accepted. The low, responsive, responsible bidder must be approved by Lincoln Hills Development Corporation. In determining the lowest responsive, responsible bidder the following will be consider: The prices contained in the bid; The ability, capacity and skill of the bidder to perform the contract or provide the service required; Whether the bidder can perform the contract or provide the service promptly or within the time specified, without delay or interference; The character, integrity, reputation, judgment, experience and efficiency of the bidder; The quality of performance of previous contract of services, including, but not limited to City contracts; The previous and existing compliance by the bidder with laws and ordinances relating to the contract or service; The sufficiency of the financial resources and ability of the bidder to perform the contract or provide the service; The quality, availability and adaptability of the commodities or services to the particular use required; The ability of the bidder to provide future maintenance and service for the use of the subject of the contract; The number and scope of conditions attached to the bid. Contractors shall not include sales tax in their bids. Owner is exempt from sales tax. Owner will provide tax-exempt certificate. A No-Lien Affidavit, fully executed and no tarized, shall be submitted with each monthly application for payment. Contractor and subcontractors shall agree to the Non-Discrimination Clause. Contractor and subcontractors shall maintain a drug-free workplace. Smoking or use of tobacco products in buildings is prohibited. (10%) participation by MBE/WBE firms in HOME funded projects
