A. The title and location of the Work is printed on the cover of this Project Manual. B. Type of Contract: Unit Price. C. Provide, where and when directed during the term of the Contract, the designated type of Abandoned Gas Well and related Work in the following counties: Western Region: Cattaraugus, Erie and Wyoming D. The quantities given on the Unit Price Schedule are solely for bidding purposes. The Director's Representative may require more or less of each estimated quantity and may require contingent Work not reflected in the Unit Price Schedule. The Work will be paid for in accordance with Section 012977 - Measurement and Payment. E. The Work at each location will be initiated by means of a Work Order. Costs will be determined on the basis of each Work Order. F. In most cases, drawings and specifications for the designated type of abandoned gas well system will be provided to the Contractor when the Work Order is issued. The Work may include any of the following items or other related work as indicated on the drawings and specifications or as directed. 1. Perform Site Inspection to determine site level of effort. 2. Provide OGS with Site inspection form and modified cost estimate. 3. Clear an access road, well pad, and perform utility locating services. 4. Mobilize all drill rig and support equipment to the well site. 5. Perform well preparation activities. 6. Perform well conditioning activities. 7. Perform wireline services activities. 8. Plug well. 9. Remove all equipment and debris and restore the site. 10. Submit Well Closure Report Properly Plug & Restore Abandoned Gas Well Sites Western Region * Design for this project commenced after January 1, 2020. Pursuant to Public Buildings Law 8(6), effective January 11, 2020, for any projects where the project design commenced on or after January 1, 2020 and for any contracts over $5,000 for the work of construction, reconstruction, alteration, repair, or improvement of any State building, a responsible and reliable NYS-certified Minority or Women-Owned Business Enterprise that submits a bid within ten percent of the lowest bid will be deemed the apparent low bidder provided that the bid is $1,400,000 or less, as adjusted annually for inflation beginning January 1, 2020. If more than one responsible and reliable MWBE firm meets these requirements, the MWBE firm with the lowest bid will be deemed the apparent low bidder.
