Per addendum 2 bid date extended to 10/28/22 FEDERAL PROJECT F.A.P. #6150(017) Town of Washington State Project No. 150-136 Rehabilitation of Bridge No. 05158, Tunnel Road over Shepaug River Contract Goal: 6% Disadvantaged Business Enterprises (DBEs)The Town of Washington (Municipality) hereby notifies all bidders that it will affirmatively ensure that in any contract entered into pursuant to this advertisement, Disadvantaged Business Enterprises will be afforded full opportunity to submit bids in response to this invitation and that they will not be discriminated against on the grounds of race, color, national origin, sex, sexual orientation, mental retardation or physical disability, including but not limited to blindness, in consideration for an award ."Bid Proposal Form", "Schedule of Prices", "Bid Bond", "Non-Collusion Affidavit" and Bidder Prequalification Approval Letter signed by the CTDOT Contracts Manager must be completed and returned with the submitted bid. Failure to properly execute and include any one of these five documents in the bid submission will result in the bid not being read and the subsequent rejection of the bid. Bidders are responsible for all printing costs and shipping costs, as applicable.Form 818 Standard Specifications and Supplemental Specifications may be obtained via the Connecticut Department of Transportation electronically All Proposals must be on the form furnished by the Town of Washington and must be requested for the above named project.NOTE: Prospective bidders must have a current sworn Statement (CON-16) on file with the Connecticut Department of Transportation and be prequalified to perform Group No. 9 Intermediate Bridges work. The BidderaEUR(TM)s Prequalification approval letter signed by the CTDOT Contracts Manager shall be included as part of the bid package submitted to the Municipality.NOTE: Any Contractor or Subcontractor engaged in surface preparation and/or coating application must be certified by the Steel Structures Painting Council.NOTE: A Surety Company Bond, on the form furnished by the Town, for at least 30 percent of the amount of the bid must accompany each proposal. A certified check will not be accepted. The Town reserves the right to reject any and all bids.NOTE: The two (2) lowest bidders, as determined by the Town of Washington immediately after the bid opening, shall submit the Pre-award DBE Commitment Approval Request form(s) to the Town of Washington NO LATER THAN FIVE (5) calendar days after the bid opening. This is a requirement of Title 49, Code of Federal Regulations (CFR) Part 26, Participation of DBEs. If the second low bidder does not submit the required DBE forms within the required time, they will not be eligible for contract award. CONTRACTORS that find discrepancies and/or errors in or between plans, specifications, quantities and other matters, must immediately notify Mr. Keegan Elder, P.E., Wengell, McDonnell and Costello, Inc., 87 Holmes Road, Newington, CT 06111, Email: KElder@wmcengineers.com in writing not less than ten days before the scheduled bid opening. The Town of Washington is an Affirmative Action/Equal Opportunity Employer. Two Hundred Forty-Four (244) calendar days will be allowed for completion of all work as described in the contract documents for Federal Project No. 6150(017), State Project No. 0150-0136 and the liquidated damages charge to apply will be One Thousand Six Hundred Dollars ($1,600.00) per calendar day.
