Remodeling and renovation of a museum in Jasper, Indiana. Completed plans call for the remodel of a 3,400-square-foot museum; and for the renovation of a museum.

Any bid received later than the above stated time will be returned unopened. Bids shall be delivered in a sealed envelope and shall be clearly marked "Bid for Jasper Arts Center - Restroom and Lobby Remodel", and marked with the name and address of the bidder. Bids must be filled in ink or typewritten. Bids will be publicly opened on said date shortly after. The work consists of restroom and lobby remodel consisting of +- 3,400 sq. ft. Restroom and Lobby Remodel, located at 951 College Avenue Jasper, IN 47547. The Project Work consists of General Construction; Heating, Ventilating and Air-Conditioning; Plumbing; Electrical; and furnishing labor, materials, and related work A combined bid for the project as shown on the plans and called for in the specifications, is to be submitted on the State Board of Ac counts Form #96, Bid for Public Works. This form must be filled out in its entirety. Also, to be submitted is the form "Work Schedule, Addendum Acknowledgment, and Alternate Bid Sums". The bidder must file with their sealed bid, a certified check or a bid bond in the amount equal to five percent (5%) of the total bid, said bond provided by an insurance company authorized to do business in the State of Indiana, payable to City of Jasper, Jasper, Indiana.The above check or bond is to insure the execution of the contract on which such bids are made. The contractor to whom the work is awarded will be required to furnish, before commencing work, a performance, maintenance and payment bond in an amount equal to the bid price of the contract awarded to said contractor, and certificates of all insurance required in the specifications. No bidder may withdraw their bid for a period of forty five (45) days following the date set for the receiving of bids. The Owner reserves the right to retain any and all bids for this period and said bids shall remain in full force and effect during said time. The Owner further reserves the right to waive informalities, and the right to reject any and all bids. The following fully executed documents shall be submitted with the bid: a. Indiana Legal Employment Declaration b. Affidavit Regarding Non-Responsibility c. City of Jasper Anti-nepotism Affidavit d. State Board of Accounts Form #96, Bid for Public Works including non-collusion affidavit with Work Schedule, Addendum Acknowledgement, and Alternate Bid Sums Form e. 5% Bid Bond or Certified Check f. Certified financial statement containing the information required in Form 96. 0900 LEGALS containing the information required in Form 96. g. List of similar past projects. h. List of sub-contractors that contractor proposes to use on this project Award will be made to the best, lowest, responsible, and responsive bidder based on the total of the base bid amount and any alternate bids accepted. The best, lowest, responsible, and responsive bidder must be approved by City of Jasper. In determining the "lowest, responsible, and responsive bidder" the following will be considered: - The prices contained in the bid; - Bidders proposed work schedule; - The ability, capacity and skill of the bidder to perform the contract or provide the service required; - Whether the bidder can perform the contract or provide the service promptly or within the time specified, without delay or interference; - The character, integrity, reputation, judgment, experience and efficiency of the bidder; - The quality of performance of previous contract of services, including, but not limited to City contracts; - The previous and existing compliance by the bidder with laws and ordinances relating to the contract or service; - The sufficiency of the financial resources and ability of the bidder to perform the contract or provide the service; - The quality, availability and adaptability of the commodities or services to the particular use required; - The ability of the bidder to provide future maintenance and service for the use of the subject of the contract; - The number and scope of conditions attached to the bid. All applicable laws, ordinances, and the rules and regulations of all authorities having jurisdiction over construction of the project shall apply to the project throughout. ' 0900 LEGALS shall apply to the project throughout. The plans and specifications are on file and may be examined at the following locations: Universal Design Associates, Inc., 910 Main Street, Ferdinand, Indiana 47532 City of Jasper, City Hall, 610 Main Street, Jasper, Indiana 47547. The plans and specifications together with all other necessary forms and documents will be available online through the project website. Only prime contractor's wanting to bid through the project website shall contact Universal Design Associates, Inc. for a link to access the portal. All questions relating to the plans and specifications shall be addressed to Universal Design Associates, Inc., Ferdinand, Indiana, phone: 812-367-2831 or email: design@udassoc.com . Contractors shall not include sales tax in their bids. Owner is exempt from sales tax. Owner will provide tax-exempt certificate. A No-Lien Affidavit, fully executed and notarized, shall be submitted with each monthly application for payment. Contractor and subcontractors shall agree to the Non-Discrimination Clause. Contractor and subcontractors shall maintain a drug-free workplace. Smoking or use of tobacco products in building is prohibited. Construction to begin on December 5, 2022. . Complete Major Components of Construction by February 3, 2023. A performance is scheduled on February 5, 2022. The lobby will need to be constructed to a state of cleanliness and safe for patrons to access the auditorium thru the lobby space. Restrooms will need to be functional at this time. . Substantial Completion of all work by February 21, 2023. A performance is scheduled on February 22 and 23, 2023. . Punch List Items Completed by February 28, 2023. . Owner is willing to work with contractor on long lead time items if they would cause issues with not completing the project by February 28, 2023.
