The Lead Abatement Services to be procured through this contract are required to fulfill the Agency's programmatic responsibility to ensure that residential buildings comply with applicable housing code and other laws. HPD has neither sufficient staff with the requisite expertise, nor the equipment necessary to perform the required volume of this type of work. The description of this procurement and location of the work for which bids are requested (the "Work") are specified in Attachment 1, the Bid Information Summary Sheet ("Attachment 1"). ARTICLE 2 SUBMISSION AND RECEIPT OF BIDS 2.1 All bids must be completed in the City of New York (the "City") Procurement and Sourcing Solutions Portal ("Passport") on or before the Bid Submission Deadline specified in Attachment 1. A. Where the Department of Housing Preservation and Development ("HPD") requires or permits the submission of paper copies of the bid or any part thereof, then such paper copies must be submitted in a sealed envelope marked with the name and address of the person, firm or corporation presenting it, the bid opening date and time specified for receipt of bids, the solicitation number and bid title. The bid and all other documents requiring signature must be signed and notarized. B. Sealed bids must be received by HPD on or before the Bid Submission Deadline specified in Attachment 1, at the location specified therein. 2.2 All properly submitted bids shall be publicly opened and read aloud at the time and place specified in Attachment 1, in the presence of the Commissioner of HPD or his or her representative (the "Commissioner"), and of any bidders who may desire to be present. ARTICLE 3 BID SUBMISSION REQUIREMENTS 3.1 The following forms or affirmations are to be completed within Passport and submitted as part of the bid: A. The Bid Forms (Item Grid and Total Bid Page); B. Questionnaire, including i. The Bidder Qualification Statement; ii. Iran Divestment Act Compliance Certification Form; and iii. Tax Affirmation 3.2 The following forms shall be obtained by the bidder awarded the Contract (the 'Contractor") from HPD's Office of Equal Opportunity: A. Employment Report (if bid is in excess of $1,000,000); B. Contract Certificate (if bid is less than $1,000,000 and more than $750,000); and C. Executive Order No. 89 (as revised) Forms and Affidavit (if bid is in excess of $250,000). 3.3 Non-compliance with these bid submission requirements may result in the disqualification of the bid. A pre-bid conference shall be held as set forth in Attachment 1. A. Bidder shall note if the Department has indicated on Attachment 1 whether a pre bid conference and/or a site meeting is mandatory; a bidder's failure to attend any mandatory meeting shall constitute grounds for the rejection of its bid. 6.2 At least five (5) City working days before the date of the pre-bid conference bidder shall notify the Agency Contact Person listed in Attachment 1 of the number of representatives that will attend the pre-bid conference on its behalf. 6.3 Nothing stated at the pre-bid conference shall change the terms or conditions of the Invitation to Bid unless a change is made by written amendment as provided in the PPB Rules. ARTICLE 7 QUESTIONS REGARDING THE INVITATION TO BID Any questions or correspondence relating to this bid solicitation shall be addressed to the Agency Contact Person designated.
