A certified check representing five percent (5%) ot the proposal or a minimum of $15,000, drawn on a solvent bank, and payable to the Mahoning County Treasurer, or a bid bond in the sum of five percent (5%) the proposal or a minimum of $15,000 in lieu thereoj must accompany each and every proposal as a guarantee that if the bid is accepted, a contract will be entered into with the Board of County Commissioners, in the manner provided by law. A performance bond in the amount of 100% of the contract price will be required atter the award of the contract. However, if the item bid is available for immediate delivery and specified as such in the bid, a performance bond will not be reouired. Bids of Corporations not bhartered in Ohio MUST be accompanied by proper certifications that such Corporation is licensed to do business in Ohio. Attention of the Bidder is directed to the requirements that each proposal must be accompanied by a non-collusion affidavit, properly executed by the Bidder. Further, the successful Bidder will be required to execute an altidavit required by Sec.5719.042 of the Ohio Revised Code. No payment shall be made on any contract for which no such affidavit has been submitted. The Board of Mahoning County Commissioners ressrvss the right to reject any or all bids and to waive informalities. In addition, the Board at Mahoning County Commissioners reseryes the right to participate in state contracts which the Department ot Administrative Services, Office of State Purchasing has entered into for the purpose of supplies, services, equipment and certain materials pursuant to Ohio Revised Code Section 125.04, No bids may be withdrawn for at least sixty (60) days after the opening thereof. Each Bidder must insurance that all employees and applicants for employment are not discriminated against bEURcause of race, color, sex or national origin. CLEARLY INDICATE THE ITEM BEING BID, AS WELL AS THE NAME AND ADDRESS OF THE PERSON OR BUSINESS SUBMITTING THE BID, ON THE OUTSIDE OF THE SEALED ENVELOPE CONTAINING THE BID.
