Contract Items and Mandatory Requirements: Vendor shall provide Agency with the Contract Items listed below. Contract Item #1: Removal of Auger Monster Charge: Vendor must quote a flat rate that includes trip, truck, and removal of Monster system, manufacture: JWC Environmental, Model # PC2251, Serial #105459-4-1. Contract Item #2: Tear Down of Auger Monster: Vendor must quote a flat rate to disassemble the Auger monster and diagnose the unit. Contract Item #3: Labor Hours: Vendor must quote a flat labor rate per hour charge for an estimated quantity of 40 hours to install the repaired parts, reassemble the auger monster, and test to make sure the unit is repaired and is operating effectively and efficiently. Contract Item #4: Installation of Auger Monster Charge: Vendor must quote a flat rate that includes trip, truck, and installation of auger monster inside the manhole. Contract Item #5: Materials Plus Markup: Vendor must provide base rate of markup for all parts needed to rebuild the auger monster. Vendor must provide original invoices on any parts purchased under this contract.
