Site work and asbestos abatement for a municipal facility in New York, New York. Working plans call for site work for a municipal facility; and for asbestos abatement for a municipal facility.
Client Number: 09-1440 This project is a two-part project. The roof replacement will be under a separate contract. Awardee will have to coordinate with the awarded roofing vendor. Location- 244-20 Grand Central Pkwy Rosedale, NY 11426 This is an RFQ (request for quotes). This is not an IFB (bid). The Bernard Fineson DDSOO intends to award the contract to the most responsible and responsive vendor that provides the lowest cost and meets the requirements detailed in the scope of work. Upon request, vendor must demonstrate relevant experience on prior projects of similar magnitude and is subject to vendor responsibility determination. Bernard Fineson DDSOO reserves the right to reject any and all quotes. In deciding whether to or not to submit a quote, please be aware the awarded vendor must adhere to the time frames outlined in the scope of work. The awarded vendor will also be required to submit certificates of Liability Insurance and Workers Compensation Insurance as well as completed Lobbying Law Forms, and Signed Appendix A. Please be prepared to submit these in a timely fashion. The Bernard Fineson DDSOO will make no concessions to the vendor for any alleged misunderstanding or deception because of quantity, quality, character, or other condition. The Long Island DDSOOs interpretation of specifications shall be final and binding. Please note all staff working at the project site will be required to sign an attendance sheet for record purposes. This includes the residence visitors log and/or an attendance sheet provided by the maintenance department. NYS Labor law requires that all contracted employees performing maintenance, repairs or capital improvements in state owned buildings receive the prevailing wage and supplements rate for the occupation in which they are working. You may be subject to signing a prevailing wage affidavit. If this is a public work contract covered by Article 8 of the Labor Law or a building service contract covered by Article 9 thereof, neither Contractors employees nor the employees of its subcontractors may be required or permitted to work more than the number of hours or days stated in said statutes, except as otherwise provided in the Labor Law and as set forth in prevailing wage and supplement schedules issued by the State Labor Department. Furthermore, contractor must pay at least the prevailing wage rate or provide the prevailing supplements, including the premium rates for overtime pay, as determined by the State Labor Department in accordance with the Labor Law. NYS law requires the vendor to keep detailed records for each person employed including name, address, telephone number, social security number, occupational classification in which worked, hourly wage rate paid, supplements provided, number of hours worked, and actual wages paid. Certified payroll record/s MUST be submitted at the time of invoicing. Effective April 28, 2008, if this is a public work contract covered by Article 8 of the Labor Law, the contractor understands and agrees that the filing of payrolls in a manner consistent with Subdivision 3-a of Section 220 of the Labor Law shall be a condition precedent to payment by the State of any State approved sums due and owing for work done upon the project. Without submittal of certified payroll record/s, payment will not be made. If you are not licensed to perform abatement services, your quote will not be accepted. Service-Disabled Veteran-Owned Set Aside: No Minority Owned Sub-Contracting Goal: 30% Women Owned Sub-Contracting Goal: 7% Service-Disabled Veteran-Owned Business Contracting Goal: 6% Contract term: 30 days High Priority County(ies): All NYS counties Location: 244-20 Grand Central Pkwy Rosedale NY 11426 Removal of slate shingle abate roof. The vendor shall provide all labor and materials necessary to complete the work in a professional manner according to scope of work and architectural drawings if supplied. Work is to be completed within 30 days of award, unless modified below. This is a NYS Public Works project and prevailing wages are to be paid. Refer to attached specification for the following scope of work. Site must be cleaned and all materials secured at the end of each work day. Absolutely no scraps, especially fasteners, are to be left unsecured. Upon completion of removal of slate shingle and ACM containing underlayment before removal of equipment and scaffolding, contractor is to schedule a quality control and quality assurance inspection with Plant Superintendent. * No award. Project will be resolicited as two separate contracts.*
