Demolition, site work and addition to a medical facility in Puyallup, Washington. Conceptual plans call for the addition of a 530,000-square-foot, 190-bed medical facility; for the demolition of a medical facility; and for site work for a medical facility.

1.1 Purpose of the Request for Proposals The City of Puyallup is seeking Statement of Qualifications (SOQs) from qualified SEPA consultants to prepare an EIS for the Good Samaritan master plan project. The city will soon issue a Determination of Significance (DS) and seeks a qualified SEPA consultant to serve as the lead preparer of the forthcoming EIS document. 1.2 Project Background The project includes the expansion of the Good Samaritan Hospital campus, including a new patient care tower (240,000 square feet, 190 new beds), two (2) new medical office buildings (200,000 square feet), central support tower (90,000 square feet), expansion of the existing Emergency Department, added building area and infrastructure related to the campus Central Utility Plant, new off-street parking, utility improvements, street improvements, landscaping, storm water infrastructure and other improvements. Scoped areas in DS This pending EIS (DEIS & FEIS) is intended to meet all applicable SEPA statutes in adequately assessing the projected impacts and associated mitigation and alternatives. Therefore, the City's objective is to select a firm which has experience and expertise in preparing EIS's in an objective, comprehensive and timely manner. The areas which have been initially identified for discussion in the EIS include: 1. Transportation, particularly roadway network, vehicular traffic, traffic safety and parking 2. Public Services and Utilities, including sanitary sewer, storm water, domestic water availability, fire services 3. Environmental health, including noise, risk of explosion, and hazardous materials 4. Air Quality 5. Earth, including volcanic, steep slope and erosion hazard areas 6. Land Use, including aesthetics, and historic and cultural resources/preservation 7. Energy, including availability, amount required, rate of use/energy efficiency, and conservation 8. Alternatives 9.
