O. Box 630 Houma, Louisiana 70361 LICENSE NO._____ Failure to have the license number on the envelope will be cause for not opening the proposal. Consolidated Waterworks District 1, Terrebonne Parish - Work on which Base Bid proposals are invited consist of preparation of existing roof and installation of new Retrofit Roof System. Bids must be submitted on the Louisiana Uniform Public Works Bid Form furnished with the Bidding Documents. This Project shall require a Louisiana Contractors license for Building Construction and/or Roofing and Sheet Metal, Siding. All questions for the project can be directed to: Houston J.
