The undersigned Bidder, having familiarized himself/herself with the specification and the Bid documents as prepared by the City of Albany Department of Water & Water Supply, and on file in the Office of the Board of Contract and Supply of the City of Albany, New York, hereby proposes to provide the goods in accordance with the Bid Documents and Technical Specifications. The Total Bid listed on this sheet shall include all costs associated with the purchase of SODIUM HYPOCHLORITE and delivery to the Albany Filtration Plant, Loudonville Reservoir, 301 Albany Shaker Rd., Loudonville, New York, 12211. It estimated that a total of 176,000 gallons of product will be purchased during the year. The product shall be Sodium Hypochlorite as manufactured by United Mineral & Chemical Corporation, Linderhurst N.J., C.B.Lind, General Chemical Corporation, Syracuse NY, K J. Ficek, Carus Chemical Company, Ottawa, Ill. or equal manufacturer meeting ANSI/ NSF 60 standards.
