**As of January 9, 2022 , project was awarded to Rogue Industries LLC , awarded amount is $3,321,442.06 ** The City of Mayfield (OWNER) will receive proposals through Codell Construction Company (Construction Manager as Contractor) to furnish all labor and materials to complete the City of Mayfield, KY Temporary Fire Station located at Gideons Crossing Commercial Park, Mayfield, KY 42066 The work for this project is related to Severe Storms, Straight-Line Winds, Flooding and Tornadoes that occurred in Western Kentucky on December 10th and December 11th, 2021 which resulted in Presidential Declaration of Disaster dated December 12, 2021, 4630-DR-KY. This is an acknowledgement that FEMA Financial Assistance Awards will be used to fund a portion of the Project Contract. All Bidders shall comply with all applicable federal law, regulations, executive orders, FEMA policies, procedures, and directives including, but not limited to those found in the Volume One Project Manual Bidding and Contract Documents. Each bid must be accompanied by a 5% Bid Bond. The bid will be submitted in duplicate on copies of the Form of Proposal located in the Volume One Project Manual Bidding and Contract Documents. Both copies of the Form of Proposal and Bid Bond shall be placed in a manila envelope and have the Project Name and Proposed Bid Package number affixed. No Bidder may withdraw a bid submitted for a period of sixty (60) days after the date set for the opening of bids.
