NATIONAL MINE HEALTH AND SAFETY ACADEMY STATEMENT OF WORK 1.0 BACKGROUND The National Mine Health and Safety Academy (NMHSA) is an educational facility which includes a residence hall located at 1301 Airport Road, Beaver, WV. This requirement is for labor, materials, and incidentals to perform three (3) quarterly PM I (walk-around inspections), one (1) annual PM II service, and PMIII services as required by MSHA to include load bank testing and antifreeze change on up to three generators annually on the equipment listed below: o Cummins diesel generator (Model No. DFEJ-6254618, Serial No. A080151177) o Cummins diesel generator (Model No. 1250DFLC-5398, Serial No. L050865158) o Olympian natural gas generator (Model No. G-150LG2, Serial No.GXC02038) **The annual PM II service shall be performed in the second quarter (April through June) each year and consists of oil change and oil/fuel/air filter replacements. An oil analysis shall be performed in the first quarter (January through March) of each year. PMIII services as required by MSHA. 1.1 OBJECTIVES The objective of this procurement is to ensure that the three generators are in constant operational order in the event of power outages in the area by performing routine repairs, inspections, and preventive maintenance on three (3) Onan generators located at the NMHSA in Beaver, WV. 1.2 SCOPE The Contractor shall provide all labor, materials, and incidentals to perform routine repairs, inspections, preventive maintenance, one (1) annual PM II in the second quarter of the year (April-June), and PM III services as required to include load bank testing and antifreeze changes on three (3) Onan generators as outlined in Part 5 in order to maintain the generators in operational order. During the course of the base year and each option period, a total of three (3) quarterly walk-around inspections will be completed and a PM II service for one quarter. The PM II service shall be performed during the 2nd quarter (between April and June) of the year and consists of oil change, and oil/fuel/air filter replacements. An oil analysis shall be performed in the 1st quarter of the year (between January and March). The Contractor shall also be required to respond to emergency repair calls (as needed) 24 hours per day, 7 days a week. SPECIAL QUALIFICATIONS The Contractor warrants that he will have obtained all licenses and permits required by law to engage in the activities that are required in connection with the Statement of Work. The Contractor shall conform to all required safety regulations for the Academy, County, State, and the Federal Government. Any applicable confined space or Lockout/Tagout procedures shall be adhered to. 1.3 PERIOD OF PERFORMANCE The estimated period of performance shall be one Base Year of twelve months with four (4) option years as follows: Period of Performance: 01/01/2023 to 12/31/2023 0001 Perform three quarterly walk around inspections on three Onan generators Product/Service Code: J059 Delivery: 01/01/2023 0002 Perform one PMII service in June, annually, on three Onan generators Product/Service Code: J059 Delivery: 01/01/2023 0003 Supplemental service on emergency basis- hourly rate during regular hours - NTE 6 hours Product/Service Code: J059 Delivery: 01/01/2023 0004 Supplemental service on emergency basis- outside of regular hours including weekends and holidays NTE 6 hours Product/Service Code: J059 General Information Payment due date is to be calculated from the date the invoice is received in accordance with FAR 32.905 and the instructions above. The relevant invoice must be attached to the inquiry email and the subject line of the email must state "INQUIRY", as shown in the following example: INQUIRY: Contractor Name, DOL Agency, Contract Number, BPA Call or Order Number, Invoice Number, Invoice Amount The contractor SHALL NOT use the DOL electronic invoicing email address for inquiries about any invoice.
