Bids must include all charges for insurance, tolls, transportation, and warehousing. Chemical supply services and deliveries shall begin in January 2023. General Requirements - FOB Destination - Material Safety Data Sheets Required The following items are to be included in your proposal, in the order listed. Deviation from this format may render your proposal non-responsive. Documents 1 and 2 will be provided in website and shall be submitted by the responding Vendor in the system as part of the bid response. 1. Completed Proposal Acknowledgement Form. 2. Completed Bid Schedule. 3. Product Literature. Data and specification sheet for the specific chemical addressed in the bid must be supplied with the bid documents. Literature should address compliance with required AWA standards. 4. Safety Data Sheet (SDS). A SDS for the specific chemical addressed in the bid must be supplied with the bid documents. 5. NSF/ANSI 60 Certification. Information indicating that the specific chemical addressed in the bid is approved for use in potable water treatment must be included with the bid documents.
