For Fire and Security Alarm Monitoring and Security, Surveillance and Access Control Equipment Installation and Maintenance Bid security and Consent of Surety Required 1) A Disclosure Form, disclosing the name and address of the Proposer and the name and address of all employees upon whom the Proposer intends to authorize to participate in Facility tours, attend the pre-bid conference and otherwise be engaged in the review of County Facility plans and/or security protocols and procedures at the Countys Facilities. The Disclosure Form shall be in the form of a certification on the Proposers letterhead, signed by an authorized corporate officer, in accordance with the form attached. 2) A copy of the companys valid: a. New Jersey DCA Division of Fire Safety Fire Protection Equipment Contractor Business Permit; b. New Jersey Burglar Alarm License; c. New Jersey Business Registration Certificate; d. New Jersey Public Works Contractor Registration Certificate; e. CSAA Five Diamond Alarm Certificate; f. Evidence of UL Certification as a Full Service Company Underwriters Laboratories UUFX Guidelines, which includes being a prime contractor that is qualified to issue UL Certificates for alarm systems installed. g. Evidence that the Proposer has been regularly engaged in the installation and monitoring of commercial security systems for a period of at least five (5) years, including one government detention facility and one jail, prison or correctional facility having an inmate population of 500 or greater.
