Renovation of a municipal facility in Dix Hills, New York. Completed plans call for the renovation of a municipal facility.

Method of Award: This is an RFQ (request for quotes). This is not an IFB (bid). The LI DDSOO intends to award the contract to the most responsible and responsive vendor that provides the lowest cost and meets the requirements detailed in the scope of work. Upon request, vendor must demonstrate relevant experience on prior projects of similar magnitude. The LI DDSOO reserves the right to reject any and all quotes. In deciding whether to or not to submit a quote, please be aware the awarded vendor must adhere to the time frames outlined in the scope of work. The awarded vendor will also be required to submit certificates of Liability Insurance and Worker's Compensation Insurance as well as completed Lobbying Law Forms, and Signed Appendix A. Please be prepared to submit these in a timely fashion. The LI DDSOO will make no concessions to the vendor for any alleged misunderstanding or deception because of quantity, quality, character, or other condition. The LI DDSOO's interpretation of specifications shall be final and binding. Please note all staff working at the project site will be required to sign an attendance sheet for record purposes. This includes the residence visitors log and/or an attendance sheet provided by the maintenance department. Prevailing Wages and Certified Payroll: NYS Labor law requires that all contracted employees performing maintenance, repairs or capital improvements in state owned buildings receive the prevailing wage and supplements rate for the occupation in which they are working. If this is a public work contract covered by Article 8 of the Labor Law or a building service contract covered by Article 9 thereof, neither Contractor's employees nor the employees of its subcontractors may be required or permitted to work more than the number of hours or days stated in said statutes, except as otherwise provided in the Labor Law and as set forth in prevailing wage and supplement schedules issued by the State Labor Department. Furthermore, contractor must pay at least the prevailing wage rate and pay or provide the prevailing supplements, including the premium rates for overtime pay, as determined by the State Labor Department in accordance with the Labor Law. NYS law requires the vendor to keep detailed records for each person employed including name, address, telephone number, social security number, occupational classification in which worked, hourly wage rate paid, supplements provided, number of hours worked, and actual wages paid. Certified payroll record/s MUST be submitted at the time of invoicing. Effective April 28, 2008, if this is a public work contract covered by Article 8 of the Labor Law, the contractor understands and agrees that the filing of payrolls in a manner consistent with Subdivision 3-a of Section 220 of the Labor Law shall be a condition precedent to payment by the State of any State approved sums due and owing for work done upon the project. Without submittal of certified payroll record/s, payment will not be made. Subcontracting: OPWDD does not allow subcontracting on this type of work. Service-Disabled Veteran-Owned Set Aside: No Minority Owned Sub-Contracting Goal: 30% Women Owned Sub-Contracting Goal: 7% Service-Disabled Veteran-Owned Business Contracting Goal: 6% Any vendor questions not asked during the site visit must be submitted in writing to: Brooklvn.CMM.Bids@opwdd.ny.gov (within two business days of the site visit: December 29, 2022). Both questions and answers (if any) will be provided to all who attend the site visit via email by: December 30, 2022. The Long Island DDSOO Maintenance point of contact was not provided for this project. Please confirm your attendance prior to the site visit by contacting Chris Mulhall at (518) 925-3659 or via email at Chris.X.Mulhall@opwdd.ny.gov DESCRIPTION: (Replace Concrete patio and install egress walkways). The vendor shall provide all labor and materials necessary to complete the work in a professional manner according to scope of work and architectural drawings if supplied. Work is to be completed within 14 days of award, unless modified below. This is a NYS Public Works project and prevailing wages are to be paid. Refer to attached specification for the following scope of work. Site must be cleaned and all materials secured at the end of each work day. Absolutely no scraps, especially fasteners, are to be left unsecured. Upon completion of project, and before removal of equipment and scaffolding, contractor is to schedule a quality control and quality assurance inspection with Plant Superintendent SCOPE OF WORK includes, but is not necessarily limited to the following: Contractor is responsible for marking out all utilities prior to any excavation. Replace rear concrete patios and install egress walkways (approximately 940 Square feet of concrete) Remove existing concrete patios and walkways, brick barbeque, vegetation, grass, roots, stones or rocks, bricks, fencing, pavers and other obstructions in area of installation, properly compact the soil base for installation of new concrete. 10' x 10' patio surface to meet bottom of door saddle. Concrete to be properly pitched for proper run off. Top of 10' x 18' patio is to be 6" below top of 10' x 10' patio. Concrete to be properly pitched for proper run off. Install 3 drainage pipes from house gutter down spout, 2 feet past patio/walkway edge. Properly grade and pitch to prevent pooling water and erosion. Install 4 (four) inches of CCA at bottom and properly compact. Install one layer of galvanized wire mesh with chairs on top of compacted CCA. Concrete patio shall be poured four (4) inches thick and shall be a qualified premix of 4000 psi. Concrete shall be scoured, contraction and expansion joints installed according to ACI guidelines. Surface to be soft bristle broom finish. All concrete work to follow ACI guidelines. Backfill with soil and seed around all edges to prevent any tripping hazard. Diagram or sketch provided for locations and approximate dimensions. Contractor is responsible to verify all measurements and conditions. Equivalent substitutions allowed with prior approval.
