Each bid shall be accompanied by a Bid Form, Bid Bond, Designation of Subcontractors, Non-Collusion Affidavit, completed Statement of Experience, Certification of Workers' Compensation, Certification Page, DIR Requirements, Fingerprinting Notice and Agreement and Certification of Drug-Free Workplace as required by the Information For Bidders.
