Alberta Health Services ("AHS") requests your (the "Respondent") quotation for Fire Alarm Monitoring Services and Fire Alarm Monitoring Panel Replacement at various sites in the South zone and Calgary zone, all as further detailed in Section 3 of Schedule "A" (the "Product(s)" or "Service(s)"). (a) Each Respondent is to complete the entirety of Schedule "A" (including Schedule A, 2.2 Mandatory Requirements), Appendix 1 - Pricing, Appendix 1.1 - Financial Workbook, and Appendix 2 - Statement of Full Disclosure and Conflict of Interest Declaration. Schedule "A" (including Schedule A, 2.2 Mandatory Requirements), Appendix 1 - Pricing, Appendix 1.1 - Financial Workbook, and Appendix 2 - Statement of Full Disclosure and Conflict of Interest Declaration once completed, are to form the Respondent's submission to this RFQ (the "Response"). In respect of Product(s) and/or Service(s) pricing: (i) Prices shall be quoted in Canadian dollars and shall include any customs duties or tariffs, overhead, profit, insurance, and warranties. Prices shall not be subject to adjustment for fluctuation in foreign exchange rates, fuel or environmental surcharges. All prices shall be quoted exclusive of the goods and services tax (as AHS is a GST tax exempt entity and federal or provincial taxes or other similar taxes, each of which, if applicable, shall be stated separately); (ii) Prices shall be quoted inclusive of freight (prepaid) at the applicable AHS site; duty, delivery, and fuel charges included, where applicable; and (iii) Response pricing shall remain irrevocable in the form submitted by the Respondent for a period of time as described in Schedule "A" Section 3.
