An example survey report showing your work product may be requested as a follow-up. 1 - Is your firm a registered DBE, SBE, MBE, or WBE If yes, please indicate which one and confirm your registration is current. 2 - Does your firm hold an existing Master Services Agreement (MSA) with Stantec If so, please provide the MSA number and expirations date. 3 - Please describe your policy on delay of work and indicate the hourly standby time. 4 - Provide resumes of key project personnel. Project managers responsible for directing and leading surveys should be certified through the State of California, Division of Occupational Safety and Health (DOSH) as Certified Asbestos Consultants (CACs) and through the State of California, Department of Public Health (CDPH) as Certified Lead Inspector Assessors. 5 - Field inspectors should be either DOSH Certified Site Surveillance Technicians and/or CACs and Certified Lead Inspector Assessors and/or Certified Lead Sampling Technicians. Copies of these certifications may be requested. 6 - Please verify and list additional health and safety training and be prepared to provide medical clearance and respirator fit test documentation for your field staff. 7 - Please describe your experience and summarize your approach to conducting ACM, LBP, PCBs in building materials and RBM surveys at commercial/industrial properties. Also describe your experience with conducting PCB sampling in accordance with BASMAA guidelines for Managing PCB Priority Building Materials During Demolition. 8 - Supplemental reporting services may be required. Please describe your experience with preparing project specifications for abatement and submitting notification through: a. The Asbestos Renovation and Demolition Notification through BAAQMD and/or b. California Department of Public Health (CDPH) form 8552 and/or c. PCBs in priority building materials abatement plans with the US EPA Region 9. 9 - Please describe any additional value-added services or unique qualifications that Stantec should consider in evaluating your cost estimate and qualifications.
