SCOPE Improve the security of the Salomon Building on the Springfield Hospital Center campus by replacing 6 sets of double doors and frames and replace them in kind with detention rated doors, frames, and hardware. Springfield Hospital Center (SHC) is seeking a vendor to improve the security of the Salomon Building on the Springfield Hospital Center campus by replacing 6 sets of double doors and frames and replace them in kind with detention rated doors, frames, and hardware This solicitation has been designated as a Small Business Reserve (SBR); only registered SBRs may respond. Vendors cannot be awarded the contract if not designated an SBR. At the scheduled site visit, each bidder will be permitted to examine the campus and applicable building, familiarize themselves with the full nature and extent of the work and obtain answers to questions about or clarification of the contract. It is the sole responsibility of the Contractor to become familiar with the campus, applicable building, and the contents of these specifications. Failure to do so does not relieve the successful bidder from their obligations to comply with all aspects of this bid package for the amount specified in the bid. Maryland Department of Health: Springfield Hospital Center 6655 Sykesville Road Sykesville, Maryland 21784 The Procurement Officer for this solicitation: Sabrina Fields Director of Procurement, SHC 410-970-7058 Sabrina.fields1@maryland.gov The Contract Monitor for this solicitation: Jeff Barefoot Director of Maintenance, SHC 410-970-7061 Jeff.barefoot@maryland.gov The sole point of contact in the State for purposes of this solicitation is the Procurement Officer. Contract 1. The contract resulting from this solicitation shall be a fixed price contract. 2. The Department reserves the right to reduce or withhold contract payment in the event the Contractor does not provide the Department with all required deliverables within the time frame specified in the contract or in the event that the Contractor otherwise materially breaches the terms and conditions of the contract. Vendor Experience and References The vendor MUST list three references in the transmittal letter. All references shall include the name address of the reference company, hospital or organization, a brief description of the type, size, and scope of services being provided and indicate the name, title, and current telephone number and email address of a specific contact person. Transmittal Letter A transmittal letter prepared on the Bidder's business stationary MUST be uploaded on eMMA with the bid documents. The transmittal letter should be brief, but must include the following: a. Three references as described above b. "Attachment C. Bid/Proposal Affidavit" c. "Attachment H. Conflict of Interest Affidavit and Disclosure Selection Process The contract resulting from this solicitation shall be awarded to the responsible and responsive bidder whose bid meets the requirements noted in this solicitation document and is the most favorable bid price (lowest cost). DESCRIPTION OF WORK a. Visitors Entrance: Contractor will remove two (2) Sets of Bronze Kawneer Storefront Glass doors (4) and frames (2) and replace with new heavy gauge steel frames and new Bronze Kawneer Storefront glass doors to fit into the 96"H x 72" W openings. Contractor will provide and install heavy duty door closers and mortise locks (Adams Rite ASSA Abloy 4510-36-101-628) on both primary doors. On both stationary doors, the Contractor will install an electric strike that is compatible with our keyless entry system. (HES 1500 Electric Strike Body-Finish 613E Bronze). The Contractor will install standard push/pull handles on all doors. b. Employees Entrance: Contractor will remove two (2) Sets of Bronze Kawneer Storefront Glass doors (4) and frames (2) and replace with new heavy gauge steel frames and new Bronze Kawneer Storefront glass doors to fit into the 96"H x 72" W outer door opening and into the 96" H x 76" W inner door opening. Contractor will provide and install heavy duty door closers and mortise locks (Adams Rite ASSA Abloy 4510-36-101-628) on both primary doors. On both stationary doors, the Contractor will install an electric strike that is compatible with our keyless entry system. (HES 1500 Electric Strike Body-Finish 613E Bronze). The Contractor will install standard push/pull handles on all doors. c. Rear Entrance: Contractor will remove one (1) set of Bronze Kawneer Storefront Glass doors (2) and frame (1) and replace with new heavy gauge steel frame and new Bronze Kawneer Storefront Glass doors to fit into the 95" H x 89" W opening. Contractor will provide and install heavy duty door closers and mortise locks (Adams Rite ASSA Abloy 4510-36-101-628) on both primary doors. On the stationary door, the Contractor will install an electric strike that is compatible with our keyless entry system. (HES 1500 Electric Strike Body-Finish 613E Bronze). The Contractor will install standard push/pull handles on both doors. d. Loading Dock Metal Doors: Contractor will remove one (1) set of steel, detention rated doors and frame and replace in kind with new detention rated steel doors and frame to fit into the 88" H x 76" W opening. Contractor will provide and install heavy duty door closers and mortise locks (Adams Rite ASSA Abloy 4510-36-101-628) on both primary doors. On the stationary door, the Contractor will install an electric strike that is compatible with our keyless entry system. (HES 1500 Electric Strike Body-Finish 613E Bronze). The Contractor will install standard push/pull handles on both doors. e. All measurements for materials are given for estimating purposes only. Contractor will visit site and make all measurements prior to ordering any materials. f. Contractor will provide all measures in the performance of this project to ensure the safety of all occupants, visitors, staff and workmen in the project areas. g. Contractor will repair, at no additional cost to the owner, any areas damaged during this project to the reasonable satisfaction of the facility. h. Contractor will dispose of all debris resulting in removing/replacing the doors. All debris shall be disposed of in accordance with all Federal, State and local regulations. i. Scheduling and execution of the work will be done at the convenience of the facility. Work can be completed Monday through Friday between the hours of 7:00AM and 3:30PM. No work shall be permitted on State of Maryland recognized holidays or weekends without prior approval of the Hospital representative. j. Within two (2) business days of notification, apparent awardee must provide for all persons coming to campus in relation to the contract resulting from this solicitation proof of COVID-19 vaccination or documentation of a medical or religious exemption approved by Maryland Department of Health's (MDH) Office of Equal Opportunity Programs (OEOP) DRAWINGS AND ATTACHMENTS a. "Attachment C. Bid/Proposal Affidavit" (to be submitted with bid submission) b. "Attachment H. Conflict of Interest Affidavit and Disclosure" (to be submitted with bid submission) c. "Additional Terms and Conditions" is attached and included as part of this contract agreement. d. The two-page facility Construction Guidelines are attached and are included as a part of this contract agreement. Special note should be taken to Item 10. All persons associated with this project shall be bound by the confidentiality requirements. No information regarding patients shall be removed, copied or discussed e. Campus Map f. Salomon Building Floor Plan g. Sample Small Procurement Contract (contract to be completed upon award) h. "Attachment N. Contract Affidavit" (to be completed upon award) i. No Bid Notice/Vendor Feedback Form Completion Date: The term of this Contract shall be for the period of February 20 , 2023 through May 20 , 2023.
