Contractor shall provide water backflow assembly testing at the Sheridan VA Medical Center in accordance with the Statement of Work. All correspondence and offers must be sent to: Jerry.danso@va.gov. Offers are due by 1pm MT on 01/27/2023. U.S. Dept. of Labor Wage Determination # 2015-5489 , Rev 20 is applicable to this acquisition and resulting contract. See Section C6 for Subcontracting Plan. Failure to complete subcontracting plan will be deemed noncompliant to the RFQ. The FORM must be filled-out and signed by all vendors. All questions must be received by 1:00 PM MST on 01/23/2023 An amendment will be issued to provide response by 01/25/23
