Questions regarding this specification can be directed to Jeff Siwiec, Director of Planning & Development at jsiwiec@ymhaonline.com All questions shall be submitted by end of business February 27 Youngstown Metropolitan Housing Authority (YMHA) invites you to submit a sealed bid proposal for general maintenance and repair at various housing sites in Mahoning County, OH. Period of contract is for two years. A two-year option period is also specified, if agreed to by both parties. Total contract shall not exceed four years. Annual budgeted project spending for this contract is expected to be $100,000, however, there is no guarantee of minimum contract work under this solicitation. This is a federal government funded contract and Federal Regulations are required to be followed. Work classifications are considered as Routine Maintenance, which qualifies this contract to be assigned under HUD Determined Wage Requirements. Wages Requirements are listed on the enclosed Form 4750 All proposals must be made on the form provided and include the following: A) Contractor Profile Form B) HUD 5369-C C) Non-Collusive Affidavit D) Completed Original Bid Form E) Statement of Qualifications with References on Contractor's Letterhead Pre-bid meeting will not be held for this contract as work is typical and is described more later in this solicitation. All bidders shall be aware that the successful bidder will be required to complete all necessary forms to meet YMHA and DHUD requirements. Contractor will be required to submit necessary paperwork and certifications including, but not limited to contract, notice to proceed, contractor profile form, authorization for direct deposit, W-9, etc. Contractor will be required to provide proof of Workers' Compensation Insurance and adequate levels of General Liability Insurance $1,000,000 and Vehicle Insurance $1,000,000 Proposals may be withdrawn at any time prior to award of contract. Contract and notice to proceed date will be April 1, 2023 and end March 31, 2025. Optional period will extend contract to March 31, 2027. YMHA reserves the right to review and rate the qualifications, experience, bid history, and past performance of the bidders, regardless of low bid, under their current and past operating names. YMHA reserves the right to waive any informalities in bidding and reject any, or all bids. YMHA intends to award one contract, however, may award contracts to any number of firms based on proposed price, contractor capacity and location. No sub-contractors will be approved for this contract. Firms are required to employ all individuals to fulfil duties under this contract. Firms are fully expected to handle all duties covered under this contract. YMHA has a goal to award 25% of all contracts to disadvantaged business enterprises (DBEs).
