1.0 SCOPE OF SERVICES REQUIREMENTS
The Supplier shall perform these services as per direction from the respective Purchaser, Provincial Health Services Authority (PHSA) Site Manager or designate, and as set out in which are the detailed directions provided by site. These services may generally encompass the following work (the "Services"): Salt all drive lanes, fire lanes, parking areas and sidewalks at the Facility rock salt shall be used in drive lanes, fire lanes and parking lots; calcium chloride shall be used where directed.
Plow drive lanes, fire lanes and parking lots and plow or shovel sidewalks and paved courtyards. Snow removal shall be pursuant to the respective site's Snow Removal Guidelines and Engagement Protocols. The Supplier's unit prices for materials and hourly rates for equipment should be included in the total Quotation. Throughout the term of this Agreement the Supplier shall maintain insurance in accordance with the requirements set forth separately. All time, materials and labour will be recorded for payment and validation on the Snow Removal and De-icing Activity Form. Snow Removal for Site Specific Services:
o Parking Lots and Sidewalks are to be cleared of snow by 7:00am
o Drive through plows and sanding to continue during the day when snowing
o The Service Provider will monitor snow levels 24/7
o Salt or Ice melt is to be used if temperature drops to 0 degree Celsius or below (without snow)
o All exterior areas within service contract scope are to be de-iced with salt or ice-melt pre-approved by site manager
o Consideration is to be given to any noise concerns of the hospital, owner reserves right of work stoppage for reasonable cause
o Availability all day if additional snow removal or de-icing service required
o Available 24 hours a day, 7 days a week to provide services
o Necessary back up equipment to enable service must be available as required Provincial Health Services Authority (PHSA) (the "Purchaser"), invite interested suppliers to submit quotations ("Quotations") for the supply of the products described in Appendix A (Services Specifications) for an initial term of one (1) year with the option to extend for up to six (6) additional terms of one (1) year each dependent on funding availability for each additional year, by completing Appendix C (Quotation Form) 2.0 SPECIFICATIONS
The specifications of the Services are set out in Appendix A (Services Specifications). Suppliers should submit Quotations that meet or exceed these minimum specifications for delivery of the Services to Richmond. 3.0 TERMS AND CONDITIONS (i) Quotations will be subject to the terms and conditions of the form of contract and award requirements set out in Appendix B (Contract and Award Requirements) (the "Terms and Conditions").
(ii) Delivery requirements for the Products will be provided in any Purchase Order issued as a result of this Invitation to Quote ("ITQ").
(iii) Respondents are strongly discouraged from including their own terms and conditions in Quotations. PHSA reserves the right to reject a Quotation that contains any such respondent terms and conditions or any other terms unacceptable to PHSA or any other Purchaser described in this ITQ. 4.0 INQUIRIES
All inquiries concerning this ITQ, including without limitation questions related to discrepancies, omissions, ambiguities and conflicts should be made in writing to the contact person designated on the first page of this ITQ ("Contact Person").
