Renovation of a municipal facility in Avon Lake, Ohio. Completed plans call for the renovation of a municipal facility.
All work shall be in accordance with the specifications located in this bid package. All bids shall be firm and shall contain the full name of each person or company interested in the bid. Bidders SHALL use the proposal form included in the contract documents in submitting their bids. Bid blanks and specifications may be secured at the office of said Board. A certified check or bid bond payable to the City of Avon Lake, Ohio in an amount equal to 10% of the total bid pursuant to Revised Code Section 153.54 as a guarantee, shall accompany the bid to ensure that if a contract is awarded it will be duly entered into and secured. Should any bid be rejected, such check will be forthwith returned to the bidder, and should any bid be accepted, such check will be returned upon the proper execution and securing of the contract. Checks of unsuccessful bidders will be returned promptly on the award of a contract to the successful bidder. The successful bidder must provide a copy of the certification issued to the bidder by the State of Ohio, Industrial Commission and Bureau of Worker's Compensation for the period covered by the contract. The successful bidder must also provide the Law Director a copy of their liability insurance holding the City of Avon Lake and the Board of Municipal Utilities harmless during the period of the contract. The successful bidder must also provide an affidavit that all indebtedness of such contractor on account of material incorporated into the work, or delivered on the site of the improvement and labor performed has been paid, according to Revised Code Section 5575.04. The successful bidder must also provide an affidavit demonstrating compliance with Revised Code Section 3517.13 (non-collusion - political contributions). The successful bidder must provide a notarized statement that at the time of making the bid that the bidder is not charged with any delinquent personal property taxes on the general tax list of personal property of Lorain County, according to Revised Code Section 5719.042. The successful bidder shall comply with the Civil Rights Act that prohibits discrimination in hiring due to race, color, national origin, religion, sexual orientation or sex. Contractors who pick up a bid packet should immediately provide the Board of Municipal Utilities with their name, address, email and contact information should it be necessary for the Board to amend any terms or conditions of this bid packet and consequently, adjust bid dates/times upon proper public notice. The Municipality, acting through said Board, reserves the right to reject any and all bids, or to increase or decrease or omit any item or items and to waive any informality in the bid at the discretion of said Board.
