Bigstone Health Commission is selecting a service provider for the repair of our building. Our objective is to select the vendor who most fully meets the requirements identified in this Request for Proposal (RFP). Bigstone Health Commission may elect to award a contract pursuant to the RFP. Your company is invited to take part in this process and provide a proposal that satisfies Bigstone Health Commission requirements. Within the RFP you will find all the information necessary to do a proper assessment Bigstone Health Commission requirement. Service providers are invited to respond to this RFP by describing how they can meet the requirements set out in this RFP and by providing other information requested. Responses to this RFP will be used to qualify service providers for the final evaluation and price negotiation phases of the selection process. The building is a single-story health centre, built of wood frame construction with concrete foundation, beams and piles. the following conditions are required to be prepared: 1. The erosion of the ground and backfill of the crawl space has occurred and accumulated over time. 2. The sump pump that is located within the area requires replacement. 3. A small trench running from the wood frame pony wall on ATTACHMENT A, as highlighted in the attached drawing. The dimensions of this trench are 6 -12 wide and 4 to 6 deep. Water is draining along the foundation and eroding the soil which created more space for the water to occupy. 4. Broken roof gutters replacement that drain the water directly on the exterior grade which is depressing into the crawl space. 5. Piles have started some deterioration in the eroded areas which could affect their strength capacity require replacement. 8. There is a concern over the eroded portion of the perimeter beam which left an opening that caused heating issues for the crawl space requires replacement. 9.
