Remodeling of a medical facility in Laramie, Wyoming. Conceptual plans call for the remodel of a one-story above grade medical facility.
For Remodel/Renovation of the First Floor and Expansion The Board of Commissioners of the County of Albany, Wyoming ("County"), by and through the Albany County Engineer, is seeking proposals from qualified individuals and firms for architectural and engineering services for the remodel/renovation of the first floor and expansion of its existing building at 3821 Beech Street in Laramie, Wyoming. Pre-Qualification:Yes The intent of this project is to consolidate County services, with the exception of Road and Bridge, court, and law enforcement related services and functions, in a single location by providing adequate space for existing services as well as allowing room for future growth. County Services that may utilize the building may include County Clerk, County Treasurer, County Assessor, Human Resources, Grants, Planning Department, Fire Warden, and others. Additionally, the Beech Street location has space to develop adequate parking for County employees and members of the public utilizing County services. 4.1. Provider's detailed scope of work to include timelines. 4.2. A price proposal including hourly billing rates, reimbursable expenses, and price schedule for each firm/sub-consultant involved shall be included for each phase. A maximum price quote for the work must be stated. 4.3. A second price proposal may be submitted for any proposed alternatives and shall be submitted in similar format to the original price proposal. 4.4. Brief description of Proposer including current workloads, goals, work, and/or projects and the ability to fulfill the requirements of this RFQ if selected. 4.5. If Proposer is an entity, include the date of formation, current tax status, and Certificate of Good Standing from the Wyoming Secretary of State. 4.6. A statement of Proposer's acknowledgment of its ability to meet the requirements of Section 2 above or identification of the specific items Proposer is unable to comply with and the reason for being unable to comply with each specific item. 4.7. A statement of Proposer's acknowledgment and acceptance of the terms of Section 6; or identification of the specific provisions Proposer is unable to comply with, the reason for being unable to comply with each specific item, and any proposed changes. 4.8. Qualifications and experience of key personnel including name, training, and experience and which area(s) of the project he/she will be assigned to. 4.9. Description of past performance (including three references) in the past five (5) years for architecture and engineering services which are most similar to this project including estimated costs and final costs for each project. 4.10. A copy of any required professional licenses for the firm and all key individuals who may be assigned to the project. 4.11. Insurance Certificates for amounts required herein. 4.12. Certificate of Wyoming residency. 4.13. The SOQ shall be signed by an officer of proposer's firm with the authority to commit the firm. The method of contractor selection has not been determined at this time
