Renovate portions of the Community Living Center (Building 217) in accordance with the project Statement of Work, drawings, and specifications. The work includes constructing a new enclosed Dementia Unit in the first-floor B-section; renovating the second-floor dining room; renovating Equipment Room B178, Nourishment Room 2B41, and Family Room 2B78; converting Rooms C104, C178, 2C04, and 2C78 into High Fall Risk Rooms; converting Rooms B149 and C149 into Bariatric Rooms; converting Rooms 2A12 and 2A13 into a large group room with a collapsible wall; demolishing the front entrance playground and reseeding the lawn; replacing all remaining flooring throughout Building 217; and installing 18-gauge sheet metal behind the walls of all perimeter rooms. Construction shall be phased to minimize interference with daily Community Living Center operations. The scope includes all labor, management, equipment, and materials required to complete the renovations. The project is divided into three separate CLINs, each with sub-phases, and each CLIN shall be priced and phased separately. All measurements and dimensions are approximate and must be verified by the contractor. Prior to commencing work, the contractor shall inspect the project area, identify any pre-existing damage, and submit an Existing Site Conditions report with photographs for Contracting Officer review and approval. If performed consecutively, the total period of performance is 740 calendar days from issuance of the Notice to Proceed.
