City of Central seeks a qualified contractor to furnish all supervision, labor, materials and equipment necessary to plant and maintain trees on the five sections (islands) of median on Sullivan Road between Lovett Road and Wax Road ("Project"). 1. Contractor shall install 57 trees in accordance with the specifications (Exhibit A) o 23 Sweetbay Magnolia (Magnolia Virginia) o 34 Chinese Fringe (Chionanthus retusus) If Chinese Fringe cannot be obtained Grancy Grebeard/American Fringe (Chionanthus virgincus) may be substituted. The substitution must be noted on the proposal. 2. Contractor shall provide all materials necessary for installation including but not limited to all trees, amending soil (native soil, topsoil, fertilizer), mulch and stakes as needed. 3. Contractor shall water each tree as appropriate for the weather conditions and shall warranty the life of all trees for ONE year following completion of installation of all trees. Warranty shall include replacement and installation without cost to Central. 4. Proposers shall provide pricing for Project: a) using 30 gallon size; and b) alternatively, using ball root. Each tree height must exceed seven feet when planted. 5. Contractor shall be responsible for removal and disposal of debris. 6. Contractor shall maintain the medians in a safe and visually pleasing condition during the installation and maintenance period. 7. Contractor shall restore any damage to medians caused by Contractor during the installation and maintenance. 8. Perform all work in accordance with all applicable laws, codes and regulations inclusive of all permits and inspections required by federal, state and local jurisdidions. Proposal shall include: 1) A copy of current Louisiana arborist or landscape horticulturist license 2) Proof of the following insurance: o $1,000,000 of commercial general liability o $1,000,000 per occurrence for bodily injury, personal injury and property damage o $1,000,000 per occurrence for owned, non-owned; leased, hired or borrowed automobiles o Workers compensation as required by law 3) Evidence of authorization to do business in Louisiana. 4) A list and description of previous experience and expertise with tree planting services at a scale comparable to the Project. 5) The name and contact information for three references who can verify proposer's experience. 6) Proposers shall provide a proposed schedule for Project assuming contact execution within seven days of receipt of proposals and completion within 45 days of contract execution. 7) A Cost Proposal on the form attached as Exhibit B. Proposers are solely responsible for all costs incurred in preparation and submission of proposals. This proposal does not commit Central to enter into a contract. Central reserves the rights to: o Select any proposal determined to be in the best interest of Central other than the lowest priced offer o Reject any and all proposals o Reduce or increase the scope of Project o Issue a subsequent Request for Proposals for same or similar Project o Approve or disapprove the use of particular subcontractors
