Interested firms shall submit a Proposal of Bid to Benjamin W. Brown, Building Commissioner, submitting three (3) copies of the following information: a. Firm name, address, telephone number, fax number and email address; b. Name of key personnel, including managers and supervisors in charge; c. Year established and any former names of operation; d. Number of years the firm has been doing this type service; e. Certificate of Good Standing/Full Force & Effect from the Ohio Secretary of State dated within 30 days of submissions of the proposal; f. Number of staff available for assignment; g. Outside services possibly used for sub contracted work; h. Two references (which shall not include the City of Warrensville Heights, Ohio), including contact information, and list of similar services provided; i.Summary of Qualifications; j. Rate per cut and property clean up; k. Name, location, and date of all its agreements for like services that have been terminated, canceled, or suspended prior to completion of the engagement or expiration of the engagement or expiration of the full term within the past five years, and any judgment terminating, or any pending lawsuits or unresolved claims or disputes for damages or termination of such agreements within the past five years; and l. Bid Bond, or certified or cashiers check payable to the order of the City of Warrensville Heights in an amount of $1,000.00. For further information, interested parties should review the Advertisement, Bid and Specifications. Proposals will be evaluated by a selection team consisting of City of Warrensville Heights staff. The bid bond, or a certified or cashiers check, as the case may be, will be held as a guarantee that, if the Proposal of Bid is accepted, a contract will be entered into between the bidder and the City of Warrensville Heights, and the performance of such contract secured by said check or bond, and default thereof said check and/or bond, and the amount represented thereby, shall be forfeited to the City of Warrensville Heights, as liquidated damages in the event the contract is terminated early.
