Bids shall be submitted on the forms provided, in sealed envelopes plainly marked as to the contents, and shall include the name of the bidder. Emailed, faxed, late, telephoned or texted bids will not be accepted. Bids will be tabulated and a recommendation will be made to the Board of County Road Commissioners at their next regular meeting. See enclosed list of roads and estimated quantities. PREQUALIFICATION - The Contractor shall be MDOT prequalified for the application of traffic marking material and show the ability of its personnel and equipment to satisfactorily apply traffic marking material. Unless specified, materials and application shall be specified in the "2020 Michigan Department of Transportation Standard Specifications for Construction." Pavement marking materials shall be selected from the MDOT Qualified Products List or approved by the Benzie County Road Commission. INSURANCE REQUIREMENTS - Throughout the time work is being performed under this Agreement, Contractor shall obtain and maintain public liability insurance in the sum of not less than ONE MILLION and 00/100 DOLLARS ($1,000,000.00) for damages relating to any one person or for damages relating to any one occurrence. This insurance policy shall name the Benzie County Road Commission (BCRC) as an additional named insured and shall contain a provision that the policy cannot be terminated, canceled, or substantially altered without thirty (30) days written notice to the Road Commission. Contractor shall provide notice of compliance with this insurance provision before beginning the work to be performed under this Agreement. INDEMNIFICATION - Contractor shall indemnify and hold harmless the Benzie County Road Commission (BCRC), its officers, board members, assigns, agents, servants, employees, and insurance companies from any and all damages, legal fees or expenses, losses, claims and actions which may arise out of performing the services contemplated by this Agreement. VERIFICATION OF APPLICATION RATES OF PAVEMENT MARKING MATERIALS - The following procedures may be used as a guideline by the Benzie County Road Commission (BCRC) to verify yields used in its pavement marking operation. Failure to comply with these guidelines will result in delay of payment until required documentation is received and accepted by an official of the Benzie County Road Commission. Each segment shall be listed in a manner so as to be relocated for inspection, mapping and billing. The log shall be used for billing and shall be emailed to Tyler Machiela at foreman@benzieroad.net at the end of each work day. This bid is for supplying the above-described materials and services to the Benzie County Road Commission from the date of acceptance through October 31, 2023. The Road Commission reserves the right to extend these unit prices for an additional period of time of up to 12 months beyond that date by mutual consent of both parties.
