A. The Work of Project is defined by the Contract Documents and includes, but is not limited to, the following 1. Project consist of completing the identified parking garage structure, once foundation work is complete, and precast is erected. Project also includes temporary utilities and coordination with Owners Single Source contractors to assist in facilitating their work. Refer to contract documents for extents of work previously performed, work required under this contract, along with design requirements and other Work indicated in the Contract Documents. B. Type of Contract: 1. General Construction work will be performed under a single prime contract with additional prime contracts for Mechanical, Electrical, and Plumbing/Fire Protection work(to be bid at a later date). INTRODUCTORY LETTER: Statement of interest and description of any proposed joint venture or prime/sub-consultant arrangements between two or more firms within a proposed consultant team, including a general description of the roles that the consulting firms will serve in the project delivery process. TABLE OF CONTENTS: Provide a clear identification of the SOQ content by section and page number. CONSULTANT REPRESENTATIVE: Name, title, location and contact information for lead person representing the firm or joint venture who is the consultant representative for the Statement of Qualifications COMPANY INFORMATION: Name, address, year established, location of branch offices, general service areas, approximate number of employees, and brief history of the firms included in the SOQ. CONSULTANT PROFILE: Identification of the specific consultant services categories to be provided by the firm/team, the basis of the firm's/team's interest for each of the service categories for which they wish to be considered, and a description of experience, technical abilities, resources, and other information that establishes the ability to provide each service, including how work will be coordinated to assure integration and control. CONSULTANT RESOURCES: Names and resumes of key personnel who would be assigned to the project, including their education, professional work experiences, expertise, licenses and a list of example projects of similar scope to the RFQ project description. Consulting firm/team should also provide a general outline of the roles of key personnel in the project deliver process. OFFERER QUALIFICATIONS: A general description of the firm's/team's familiarity with local conditions and codes, ability to relate to the general project requirements, and availability of staff for the project. REFERENCES: List a minimum of five professional references based on a list of clients where related services were provided by staff identified in Consultant Resources above. SUBMITTALS: Statement of qualifications submittal documents should be limited to a total of approximately 10 pages
