Sealed proposals for replacement of an approximate 3,600 square foot EPDM roof, including select demo and disposal of existing, as shown and specified on the project documents for the Sanitary Board of the City of Charleston, WV, a Municipal Utility (CSB) This work consists of providing all labor, equipment and materials necessary to install, in accordance with all applicable building codes, an EPDM roof and all other appurtenances and items as shown and specified in the project documents. Upon issuance of a written Notice to Proceed, the Work shall be completed within 60-consecutive calendar days thereafter. Each Bid must be submitted in a sealed envelope containing all necessary documents including the Bid. Proposals must be made on the forms furnished by the CSB in the manner designated in the Contract Documents and must be enclosed in a sealed envelope bearing the name and address of the Bidder and the notation "Bid Enclosed" on the outside. Envelopes must have the following information in the lower left-hand corner: Name and address of Bidder, Renovation of Customer Service dministration Building Roof. The envelope containing the Bid will be opened and publicly read aloud. Bids received after the scheduled closing time for the reception of Bids will be returned unopened to the Bidders. The CSB reserves the right to reject any and all Bids and is not necessarily bound to accept the lowest bid, if said bid is contrary to the best interest of the CSB. Per the requirements of the Article 21-11-11 of the Code of the State of West Virginia, any Contractor submitting a Bid on the Project hereby certifies, indicates, and acknowledges that he he has a Contractor's license from the State of West Virginia and meets all the qualifications required by the statutes of the state and subdivision in which the Work is to be performed. Per the requirements of Article 21-ID-5 of the Code of the State of West Virginia, any Contractor submitting a Bid for a public improvement construction contract shall provide an affidavit of compliance that the Contractor has a written drug-free workplace policy. Failure to complete the Drug Free Workplace Conformance Affidavit Bid Form shall result in disqualification of the Bid. The CSB reserves the right to waive any informalities in the bidding process.
