One (1) set of bid documents, including adequate liability and property damage insurance, proof of valid workers compensation coverage, warranty coverage and all other necessary certifications/paper work (Non-Conflict of Interest Certification, Purchasing Affidavit, Drug Free Workplace Conformance Affidavit and Coversheet, Employment History Disclosure, Ethics Disclosure) shall be enclosed in a sealed envelope and marked visibly on the outside of the envelope FOUNDATION REPAIR.This envelope shall be addressed to Stevens Correctional Center, Attn: Louvinia Collins 788 Virginia Ave. Welch, WV 24801. Stevens Correctional Center nor the McDowell County Commission will not be responsible in the event that the U.S. Postal Service or any other courier system fails to deliver the sealed bid to Stevens Correctional Center by the given deadline above. No bids will be accepted via facsimile or electronic submission.
