The proposed work is located in Pinal County on State Route 387 within the Gila River Indian Reservation between milepost 8.60 and milepost 15.70. The work consists of milling and replacing the existing asphaltic concrete, shoulder buildup, pavement marking and other related work. This project is located on a Native American Reservation, in the Gila Indian Reservation area, which may subject the contractor to the laws and regulations of the Gila Indian Reservation and its TERO office. Contractors are advised to make themselves aware of any taxes, fees or any conditions that may be imposed by the Gila Indian Reservation on work performed on the Reservation. If a request for approval to bid as a Prime Contractor is received less than 48 hours prior to bid opening, the Department cannot guarantee the request will be acted on. To submit a valid bid, the bidder must (1) have prequalification from the Department as necessary for the project, and (2) be included on the project Plansholder List as a Prime. A proposal guaranty in the form of either a certified or a cashier's check made payable to the State Treasurer of Arizona for not less than 10 percent of the amount of the bid or in the form of a surety (bid) bond for 10 percent of the amount of the bid shall accompany the proposal. Surety (bid) bonds will be accepted only on the form provided by the Department and only from corporate sureties authorized to do business in Arizona. The bidder to whom an award is made will be required to execute a Performance Bond and a Payment Bond, each in 100 percent of the amount of the bid, an Insurance Certificate and the Contract Agreement. The time allowed for the completion of the work included in this contract will be 130 calendar days. The minimum contract-specified goal for participation by Disadvantaged Business Enterprises in the work, as a percentage of the total amount bid, shall be 7.30.
