Description: 3.4 SCOPE OF WORK 3.4.1. Demolition and removal of home (to include removal of concrete slab) removing all accumulation, building/construction debris taking it to an authorized landfill and remove the abandoned septic tank. 3.4.2. Quote shall be inclusive of labor, equipment, permits and incidentals necessary to successfully complete the project. 3.4.3. Property Address: 4115 Palazzo Street, Sebring, Florida 33872. 3.4.4. Parcel ID: C-04-34-28-020-0190-0170. License requirement: Demolition License, or A, B or C License is required Insurance requirements: Vendors may submit a certificate of insurance (Acord form showing the Highlands County Board of County Commissioners as the certificate holder) with their response to this Request or may submit with that Response a letter from a licensed insurance agent confirming that the vendor can be insured for the amounts required by paragraph 2.11 of this Request upon award. The awarded vendor must supply a certificate of insurance (Acord form showing the Highlands County Board of County Commissioners as the additionally insured) for the work to be performed or goods to be delivered pursuant to the purchase order issued pursuant to this Request before that purchase order will be issued.
