For LMIG Roadway Improvements Approximately 0.44 miles of asphalt resurfacing of a portion of Bell Avenue and Walnut Street and all of Mulberry Street and Hall Street. Partial removal and placement of asphaltic concrete open graded crack relief interlayer on Bell Avenue. Full Depth Reclamation (FDR) of the portion of Walnut Street and all of Mulberry Street and Hall Street. Striping as required. Permanent grassing of shoulders. Asphalt seal coating of approximately 1,500 L.F. of drives within the City Cemetery, also to include pavement patching of two locations minimum or as directed by the engineer, also to include asphalt-rubber joint and crack sealing of existing cracks. A Bid Bond will be required with proposal submission. The successful bidder shall be required to provide a Payment Bond and a performance bond, each in an amount not less than 100% of the total bid amount, at the signing of the contract. Successful bidder will be required to provide insurance in the amount of at least ($1,000,000.00) one million dollars as required by paragraph 17 of the "terms and conditions" of these documents. Proposals that do not clearly outline responses to each criterion may be eliminated from further consideration. Failure to sign your proposal in the 'Original' transmittal letter will force your proposal to be declared as 'non-responsive' and not be considered for award. No bid may be withdrawn for a period of sixty (60) days after time has been called on the date of opening.
