This project includes removal and replacement of approximately 250 square yards of concrete pavement patching at various locations throughout the village. All bid proposals offered must be accompanied by a bid bond, cashier's check, or certified check in an amount which shall not be less than Five Percent (5%) of the total amount of the proposal. All bidders wishing to obtain bidding documents must be approved by the Village prior to obtaining bidding documents. All bidders must provide proof that they are prequalified with the Illinois Department of Transportation to perform at least 50% of the value of the work before being issued bidding documents. This Project consists of removal and replacement of the existing pavement at various locations within the corporate limits of Maywood, Illinois. The scope of improvements generally consists of restoration of certain sections of pavement which have been disturbed due to emergency sewer and water repairs. The Contractor will be responsible for implementing and maintaining the proper traffic control and protection on this Project. The work shall include removing existing stone, brick, concrete or asphalt pavement, some potential dirt spoils, and installing an eight-inch (8") thick finished Portland Cement Concrete pavement patch, and any other necessary work to properly complete this project in accordance with the specifications and bidding documents. There are approximately 36 individual locations with an estimated quantity of seven (7) square yards per location. There are three (3) individual mobilizations to be performed by the Contractor.
