Each Bid must be accompanied by a sworn and notarized statement disclosing any familial relationship that exists between the owner or any employee of the contractor and any member of the Board of Education or the Superintendent of Madison District Public Schools. The Board of Education will not accept a Bid that does not include this sworn and notarized disclosure statement. Each Bid must also be accompanied by a sworn and notarized statement certifying that the contractor is not an Iran Linked Business. The Board of Education will not accept a Bid that does not include this sworn and notarized statement. Each Bid must also be accompanied by a sworn and notarized Criminal Background-Sex Offender Affidavit. The Board of Education will not accept a Bid that does not include this sworn and notarized Affidavit. Madison District Public Schools' Board of Education reserves the right to accept or reject any and all Bids, either in whole or in part, to waive any informalities or irregularities therein, and to award the contract to other than the bidder(s) submitting the best financial Bid (low bidder), in its sole and absolute discretion. Each Bid must also be accompanied by certified check, cashier's check, money order or bid bond made payable to the School District in an amount not less than five percent (5%) of the total bid amount(s). All Bids shall be firm for a period of sixty (60) days after the Due Date. Successful Bidders whose Bids are $50,000 or more will be required to furnish satisfactory Performance and Payments Bonds by a Treasury listed surety in the amount of 100% of their Bid.
