renovation of the office space located at 310 & 330 NE Kirby Street, McMinnville, Oregon 97128. Once
completed, the new structure will provide added space and capacity to support operations for HHS's
Public Health Program.
A. Site design and construction of approximately 6,045 square feet of office space on one
floor of the existing facility to match specifications
B. One story with approximately 5-7 new rooms including 2 exam rooms, a medical lab
space, a lobby and potentially the division of larger offices into several smaller rooms. Remodel
of existing rooms to include removing walls, flooring, installing plumbing for a sink and
ensuring necessary electrical/plumbing/ventilation capacity for large medical equipment (i.e.,
ultra-cold freezer, autoclave, multiple vaccine refrigerators). Carpet and paint throughout.
C. All permits, landscape design, parking and other requirements to be met with City of
McMinnville and fees are to be included in the proposal
D. Completion of all work in accordance with all applicable federal, state, and local
requirements
E. Value engineering ideas are welcome as long as items are equal in quality and function
F. Design and construction - Acquisition of all necessary permits and approvals
G. Connections to existing systems
H. Coordination with utilities
I. 12-month warranty on construction work
J. Local, state and federal compliance as required
