This report prepared by (CML) of Jackson, Tennessee, detail the subsurface geotechnical investigation for the proposed HCCHC Women's and Children's Clinic in Bolivar, Tennessee. This report contains a description of the site conditions and foundation recommendations based on the subsurface drilling. Based on current information, the proposed building will be an approximately 7,460 one story building with pre-fabricated wood trusses wood studs continuous footings and a floor slab on grade. Parking areas will be on the south and east sides of the building. Although the exact loading is unknown, conservative estimates of maximum loads for the building are approximately 1-2 kips/ft on wall footings and 125-150 pounds per square foot (psf) on floors labs.
